    PLDT Home Refund Form Privacy Notice


    PLDT, Inc. (“PLDT”) respects your fundamental right to privacy and we commit to take great care in safeguarding your
    personal data. In compliance with the Data Privacy Act, we share with you the principles that will govern how your personal
    data will be collected and processed when when you use the “PLDT Home Refund Form”.

    Why We Collect and Process Your Personal Data


    The PLDT Home Refund Form allows PLDT Customers to request for a refund for the advanced monthly service fee
    (“AMSF”) paid to PLDT under the scenario where the application is cancelled before service installation.

    Under an AMSF refund request, we collect and process your personal data under the following circumstances:

    Refund Request
    For PLDT to facilitate a refund request, we will ask for your (a) PLDT Account Number, (b)
    PLDT Account Name, (c) your mobile number, (d) your e-mail address, (e) the payment
    channel you used to pay for the AMSF, (f) the date when you paid for the AMSF, (g)
    the amount you paid, (h) your preferred e-wallet provider (GCash or Maya), (i) the e-wallet
    account name, and (j) the e-wallet account number.

    This information will be provided to a PLDT internal team so that your refund request may
    be validated.

    Refund Transfer
    For PLDT to facilitate a refund, it will provide the following information to G-Xchange, Inc.
    (GCash) or Maya Philippines, Inc. ("Maya"): (a) an e-wallet account name, and (b) an e-wallet
    account number.

    How Your Personal Data is Protected


    The integrity, confidentiality, and security of your personal data is important to us. This is
    why we strictly enforce this Privacy Notice within PLDT and have implemented technical,
    organizational, and physical security measures that are designed to protect your
    information from unauthorized or fraudulent access, alteration, disclosure, misuse, other
    unlawful activities, and from other natural and human dangers.

    Data Retention
    AMSF refund information will be retained for thirty days, after which, data disposal will
    follow. This disposal however will only be reflected on PLDT’s end.

    Note that PLDT AMSF refunds are disbursed through GCash or Maya. These e-wallet
    providers may also process the information you provided to PLDT for their own purposes.

    Your rights and choices
    The Data Privacy Act gives you certain rights to your Personal Data. Subject to exceptions
    under the law, you are entitled to request:
    1. Access to the Personal Data we process about you;
    2. Rectification of your Personal Data if it is found to be outdated, inaccurate, or
    incomplete;
    3. Erasure of your Personal Data in cases where it is no longer needed to achieve the
    legitimate purpose of its processing;
    4. Restriction of processing of your Personal Data in limited circumstances;
    5. Portability of your Personal Data so that you may receive a copy thereof or that we
    transmit the same to another company on your behalf.

    Depending on the circumstances, such as when we process your personal data based on
    your consent, you may also have the right to object to the processing thereof. If you believe
    that your data privacy rights have been violated, we encourage you to contact PLDT’s Data
    Privacy Office to seek resolution of your complaint. You also have the right to go directly to
    the National Privacy Commission or to make a claim against PLDT before a competent
    court
