The outpatient clinics are best accessed via the main entrance to the hospital, where
individual clinics are signposted. On arrival at the hospital please use the screens to
book in where available. Alternatively please report to the clinic reception desk.
Missed appointments costs the NHS approx £160, so if you are unable to attend this
appointment please contact us on 01623 672383 or email sfh-
[email protected] so that the appointment will not be wasted and can be
offered to another patient. If you fail to attend, we will assume an appointment is no
longer required and you will be referred back to the care of the GP. Our office hours
are 8am to 6pm Monday-Thursday, Friday 8am to 6pm and 8am to 2pm Saturday.

All our hospitals are well served by a local bus service. To find out which bus

200 2233.Do you find it difficult to access public transport? For a small annual
fee, your local social car scheme can help. To register and book journeys
contact Our Centre on 01623 753192 for Ashfield and Mansfield districts or
Ashfield Voluntary Action on 0115 9637261 for Sutton In Ashfield and
Hucknall.
Ample parking facilities are available at all of our hospitals. Please note that
the ticket machines at our Newark site are currently 'cash only'. For the latest
information about parking, including prices and the national Healthcare Travel
Costs Scheme, please look on the visitors section of our website www.sfh-
tr.nhs.uk.

any future appointments, please notify us of your mobile telephone number.

Please note, we are a research active Trust and as such you may be
approached by research staff if you are eligible to participate in any current
studies.
If you would like a member of our staff to accompany you, please ask a member of
the clinic team.
All of our hospitals are smoke free and as such you will not be able to smoke
anywhere on site including the car parks, pathways and grounds. Please
speak to a member of staff if you would like to stop smoking. We can offer
advice and refer you to services that support smokers who want to cut down
or stop completely.
