MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:


This Agreement made and entered into this 19th day of September 2022 at Dilag,
Tabuk City Kalinga, by and between:
DEPARTMENT OF EDUCATION(DepEd), -DILAG INTEGRATED
SCHOOL with office address at Dilag, Tabuk City, represented herein by the School Head,
IMELDA L. HABAN, herein referred to as “DepEd-Dilag Integrated School:”
-and-
XYCLAUZE MARTIN AGBULUS, ADDRESS OF PUROK LIBERTAD,
DILAG, Tabuk City represented herein by MANNY AGBULUS, referred to as “PARENT”.
WITNESSETH
WHEREAS, the Department of Education (DepEd) has applied distance
learning modalities to ensure learning continuity. This way, students can continue their education
in remote setups through online/offline platforms, TV and radio, and printed modules.
WHEREAS, the Department of Education (DepEd), in collaboration with its
partners and stakeholders, shall ensure that all schools are conducive to the education of children.
The best interest of the child shall be the paramount consideration in all decisions and actions
involving children, whether undertaken by public or private social welfare institutions, courts of
law, administrative authorities, and legislative bodies, consistent with the principle of First Call
for Children as enunciated in the CRC.
WHEREAS teachers and learning facilitators especially in learning centers are
their substitute parents and are expected to discharge their functions and duties with this in mind.
WHEREAS DepEd believes that one way of achieving an enhanced learning
environment and improving the quality of basic education is to enter partnerships that are able to
demonstrate a contribution to improve learning, development and wellbeing outcomes for
learners.
NOW THEREFORE, for and in consideration of the foregoing premises and the
terms and conditions herein set forth, the parties hereby agree to undertake the following:
1. Advisers shall orient students about the rules and policies of the modular distance learning.
2. Advisers shall provide signed Consent forms from the parents as applicable.
3. Advisers shall receive the modules from the different subject teachers and release them to the
learner to be accomplished within time allotted.
4. Adviser shall retrieve and distribute to the subject teachers.
5. Subject teachers shall monitor and update learning progress of the learner
6. Communicate student’s progress to the learner/parents/guardians through text, call and home-
visitations.
7. Issue a final copy of grade to the student upon completion of the requirements within a
prescribed period.
8. The student/parents/guardian shall receive modules on the scheduled dates of distributions.
9. The learner submits accomplished modules to the adviser and all other related requirements,
which will serve as a basis for the computation of the academic performance to the agreed dates
of submissions.
10. The learner will be joining the face-to-face conduct of scheduled quarterly exam.
11. The student’ failure in submitting and complying with the requirements would mean
FAILURE in the subject/grade level.

NOW THEREFORE, in consideration of the foregoing program, both parties shall strictly follow
the above terms and conditions:

IN WITNESSES WHEREOF, the parties hereto set and signed this document on this 19th day of
September in Dilag Integrated School, Dilag, Tabuk City, Kalinga.
