Notice of Monetary Redetermination

PRISCILLA HUGHES,

Your claim for Pandemic Unemployment Assistance was filed on October 23, 2020. Information about
your monetary eligibility is explained below. You should keep this letter for reference throughout the
duration of your claim.

Decision

You meet the monetary eligibility requirements to establish a claim for Pandemic Unemployment
Assistance benefits. You are entitled to receive benefits as long as you are not disqualified or held
ineligible for other reasons.

Claim Summary
• You may receive up to $267.00 each week you are eligible for benefits.
• Your weekly benefit rate is $267.00.
• Your weekly dependency allowance is $0.00.
• You are eligible for up to 81 weeks of benefits.
• Your maximum benefit amount, not including your dependency allowance, is $21,627.00.
• Your benefit year is effective from March 15, 2020 until September 4, 2021.

The maximum benefit amount shown above does not include any Federal Pandemic Unemployment
Compensation (FPUC) that you are potentially eligible to receive. Individuals receiving Pandemic
Unemployment Assistance benefits may also be eligible to receive an additional $300 weekly benefit
amount under the FPUC program between the weeks ending January 2, 2021 and September 4, 2021.

You do not need to contact the agency or submit a separate application to be considered eligible
for the additional $300 FPUC. It will be automatically applied to your payment for qualifying weeks and
is separate from your Pandemic Unemployment Assistance weekly benefit amount.

High Unemployment Period Extended Benefits (HUP EB) triggered off in Massachusetts due to economic
conditions as of the week ending February 20, 2021. Because of this, Massachusetts PUA claimants are
no longer entitled to the seven week HUP EB extension of benefits in addition to the 81 weeks.

Weekly Certifications

You must certify your eligibility every week to continue receiving benefits. Failure to certify each week
will result in non-payment for that week.

Online: To certify online, visit https://ui-cares-act.mass.gov/PUA/ and log in to your account. Your online
account is accessible twenty-four hours a day and seven days a week.
By Phone: To certify by phone, call (877) 626-6800, Monday through Friday between 8:30 AM and 4:30
PM Eastern Time.

Appeal Rights

If you filed an appeal of a previous monetary determination or redetermination for your PUA claim, that
appeal has been resolved by this redetermination and that appeal will be dismissed. You do not need to
take any further action. If you disagree with this redetermination, you may request a hearing on (appeal)
this redetermination. Your appeal must be received within 30 calendar days from the issue date of this
determination.

To file an appeal online go to https://ui-cares-act.mass.gov/PUA/ and log in to your account. Select the
"More..." tab and click on the "File an Appeal" link.

You will be asked for a Letter ID that can be found at the top right-hand corner of the decision you are
appealing. Enter the reason you are appealing in the available text box and submit the request.

If you are unable to file your appeal online, please call (877) 626-6800 for assistance.

If you are still unemployed while waiting for a hearing, you must continue to request benefit
payments each week. Even if your initial determination is reversed, you will not receive benefit
payments for the weeks when you did not request benefits.

Income Verification

If you have new or additional documentation to support your claimed 2019 income, please contact the
PUA Call Center at (877) 626-6800 for assistance. DUA may then re-determine your WBA to a higher
amount, depending on the information you provide. You may submit this documentation at any time until
September 4, 2021.

Reminder: You will only be paid for weeks that you have requested and for which you have been
determined eligible. You must request benefits for every week that you wish to be paid. If you have not
already done so, you can complete your weekly certifications by visiting
https://ui-cares-act.mass.gov/PUA/ and accessing your account.

Receiving Correspondence

Immediately notify us of any changes to your address or phone number. You can update your address,

logging in to your account.
