    Dear Respondent:
    Good day! I am a social Work graduating student of MMG College of General Santos
    City Inc, currently completing my research paper on Lived experience of person who use drugs
    who completed of community-based drug rehabilitation program in barangay Fatima of General
    Santos City. An assessment, please answer this questionnaire as honestly as possible. All the
    information obtained from this questioner will be kept confidential and will only be used for the
    purpose of this study.
