OBJECTIVES:

The main objective of this study is to provide the needed nursing care to a childbearing mother at risk of
postpartum hemorrhage. To provide advices, health teaching to the patient and family for maternal and
child care and management of possible risk factors and other obstetric complications.

For the general objectives, this study aims to:

• To obtain obstetric history of the patient;

• To perform physical assessment to the childbearing mother;

• To identify physiologic changes;

• To provide health teaching to the patient and family;

• To identify major and minor discomforts of the patient; and

• To apply nursing care process to the patient.

At the end of the case study, the learners will be able to:

• to define postpartum hemorrhage;

• to determine what causes hemorrhage during postpartum;

• to understand the physiologic changes happening in a pregnant woman at risk of postpartum


hemmorhage;

• to gain knowledge about factors affecting the recovery of a pregnant woman; and

• to fully understand and have confidence in handling similar cases in the future.

For the specific objectives, this study aims to:

• Accurately present all assessments of the patient which includes physical assessment and family
history taking;

• To understand the anatomy and physiology of the case study being presented;

• To identify the nursing diagnosis;

• To understand the role of drug in managing the client related to the diagnosis;

• To efficiently formulate appropriate nursing care plan for the problems identified; and

• To appropriately apply and exhibit nursing interventions necessary for the childbearing mother’s
condition at risk of postpartum hemorrhage.
DISCHARGE PLAN

Medicine  Continue taking Cefazolin for 5 days more.


Emergency  You are suddenly short of breath and feel
lightheaded.
 You have sudden chest pain.
 You are breathing faster than normal.
 Your heart is beating faster than normal.
Call your doctor or obstetrician if:  Your bleeding increases, or you have
heavy bleeding that soaks 1 pad in 1 hour
for 2 hours in a row.
 You have a fever.
 You pass large blood clots.
 You feel dizzy.
 You have a low back ache, abdominal pain
or tenderness, or loss of appetite.
 You are urinating less than usual, or not at
all.
 You have questions or concerns about your
condition or care.
Health teachings  Encourage patient to have a complete bed
rest.
 Educate patient to observe for the color of
vaginal bleeding especially if after 3-4
days, your flow becomes bright red again
and increases in amount, it usually means
that you have been doing too much. Rest
more.
 Emphasize the importance of perineal care
to prevent infection and increase comfort
after vaginal delivery.
 Encourage patient to drink lots of fluids
and to eat foods from all four of the Basic
Food Groups. Continue to take your
personal vitamins and iron tablets as
ordered by your doctor.
 Monitor any untoward symptoms of
infection as a complication of the surgery.
Social-Sexual Needs  No sexual intercourse for 4-6 weeks is
suggested
OPD Follow up  Patient will visit her doctor’s clinic a week
after her discharge with carried CBC
laboratory results.
