found thereat, as spouse of the late Aniceta D. Bongcas, to my son GIOVANNI D. BONGCAS,
Filipino, of legal age, married to Jennie II Vergara and a resident of Airport Road, Tagbilaran
City, Bohol, Philippines, in an Extrajudicial Settlement of Estate with Adjudication and
Simultaneous Donation executed in Tagbilaran City, duly acknowledged before a Notary Public,
Attorney Jodel Theodore C. Cabahug, CPA as Doc. No. _______; Page No. _______; Book No.
_________ and Series of 2014;

2. That aside from the aforesaid Donation, I have not made any other donation during the
year 2014; and

3. That I am executed this affidavit to attest to the truth of the foregoing facts as required
by the Bureau of Internal Revenue (BIR), for the Issuance of a Donor’s Tax Clearance to my son,
Giovanni D. Bongcas and for whatever legal purpose this may serve now and in the future.

1. That we personally know very well the late ARISTON TALIP, who died intestate and without
known debts on July 20, 1979 in Baclayon, Bohol;

2. That her death was duly registered at the Office of the Local Civil Registrar of Baclayon, Bohol,
Philippines;

3. That we learned recently, that her records of death at the Office of the Local Civil Registrar of
Baclayon, Bohol, Philippines, is not available;

4. That we know all these facts because we are close friend of the family of the late Bonifacia Talip,
although we are not related to them;

5. That we are executing this affidavit to attest to the truth of the foregoing facts and for whatever

possession and custody;

4.) That the Confirmation of Participation Nos. COP No. A 260265 and A 260266 were inadvertently
misplaced and eventually got lost at home in Airport Road, Tagbilaran City, Philippines;

5.) That I exerted effort to find them, but the same could not be found;

6.) That there is a reasonable ground to believe that the above-mentioned items could not be
found;

7.) That the Confirmation of Participation Nos. COP No. A 260265 and A 260266 are not in the
hands of any third person or of person in authority for valued consideration;
