Program/Activity Title: Family Earthquake Awareness/Preparedness

Objective/s: To provide parents information needed for reducing risk of threats from natural and human-
made/induced disasters
Program/Activity Design Date: June 22, 2018
Time: 3:00 – 4:00 P.M.
Venue: Grade 6 – Earth Room, San Jose Patag Elementary School
Key The following tasks are expected:
Results: a. Demonstration of “Drop, Cover and Hold”
b. SDRRMC Lecture
c. Open Forum and Exchange of Ideas
d. Drill inside the room
e. First Aid
Persons Involved: School Principal, Class Adviser and Parents
Expenses: School Fund
Lessons Learned/ The earthquake drill with Barangay Rescue Team and Coordinator helped and motivated the students,
Recommendation: local leaders and school personnel on how to reduced risks from the threats of natural and human
-made/induced disasters. Parents were informed on the basic preparation on the said activity. The
parents participated in the earthquake drill during the classroom orientation. The parents gave
suggestions on how to reduce risks inside the classroom for safety of the learners.

Objective/s: To conceptualize and validate a useful and innovative Strategic Intervention Materials (SIM)

Results: a. Technical Assistance
b. Handy and pupil-friendly SIM
c. Focal Group Discussion
d. Collaborative Work
e. Concept Brainstorming
Persons Involved: SIM Winners, School Principal, Class Adviser and Parents
Expenses: Personal Fund /School Fund
Lessons Learned/ Through the collaborative effort of the team, a more affordable, a teacher and pupil-friendly materials
Recommendation: can be easily developed and produced that would be a great helped to enhance and uplift slow learners
in learning Math lessons through SIM and at the same time it can easily assure the team to champion
SJPES in the forthcoming District and Division Strategic Intervention Material Contests.

Recommendation: cooperate, and be a true partner of the teacher and the school for the welfare of their children. Regular
conferences will be held as to look after progress of school policies and pupil’s performance as well.
