Product Disclosure Sheet Date: as per quotation date

Important Note
Read this Product Disclosure Sheet before you decide to take out the SmartDrive Private Car Insurance Policy. Be sure to also read through
the general terms and conditions.

This policy indemnifies you against your liabilities to third parties for injury or death, damage to third parties̏ property, and
accidental or fire damage to your vehicle or theft of your vehicle.

2. What are the covers / benefits provided?


This policy covers:

A. Loss or damage to your own vehicle due to accidental fire, theft or accident.
B. Your liability or your authorized drivers liability to third parties for:
ƒ bodily injury and death; and
ƒ property loss or damage.

Optional benefits that you may wish to purchase by paying additional premium:
ƒ Breakage of glass in windscreen, window or sunroof
ƒ Passenger liability cover
ƒ Damage arising from flood, typhoon, hurricane, storm, tempest, volcanic eruption, earthquake, landslide, landslip or
sinking of the soil/earth or other convulsion of nature
ƒ Strike, riot and civil commotion
ƒ Compensation for Assessed Repair Time
ƒ Current year NCD relief
ƒ Vehicle accessories

NOTE: It is an offence under the laws of the Republic of Singapore to enter the country without extending passenger liability
cover to your motor insurance.

Duration of cover is for one year. You need to renew the insurance cover annually.

3. How much premium do I have to pay?


The premium you have to pay may vary depending on the sum insured and cubic capacity of your vehicle, no-claim-discount
(NCD) entitlement, optional benefits required and our underwriting requirements.

Example
Gross premium for a new private car with sum insured RM40,000 and 1300 cubic capacity
Comprehensive cover: RM1,239.20
NCD entitlement (25%) RM 309.80 For actual amount, please refer
Additional cover – Windscreen damage up to RM600 RM 90.00 to quotation.

The estimated gross premium which is including agent’s commission but excluding service tax and stamp duty is
RM1,019.40.

* This sum insured is based on the current market value of the vehicle based on reference to RedBook vehicle valuation
database system.

4. What are the fees and charges I have to pay?


i. Commission to the insurance agent 10%
ii. Stamp duty RM10
iii. Service tax (for organizational policies only) 6%

5. What are some of the key terms and conditions that I should be aware of?
ƒ Duty of disclosure - You must disclose all material facts which you know or ought to know such as your personal
pursuits which could affect the risk profile. You must ensure that the proposal form is completed accurately as it forms
the basis of the insurance contract.
ƒ Cash before cover - Full premium must be paid to us or our authorized agent representing us before cover can be
granted.
ƒ Under/ Over Insurance - You must ensure that your vehicle is insured at the appropriate amount.
ƒ Compulsory excess - if your car is driven by a person who is:
Under age 21 or
 

A “L” license holder or




A full license holder for less than 2 years or


 

Not named in your policy




The amount of loss you have to bear is RM400


ƒ Claims - After an event which may become the subject of claims under the policy,
a) Please make a police report within 24 hours and immediately notify us in writing with full details
b) Betterment would be applied for your vehicle aged five years and above
c) You may not be entitled to a NCD on renewal of your policy

Note: The list is non-exhaustive. Please refer to the policy contract for the full list of terms and conditions under this policy.

6. What are the major exclusions under this policy?


This policy does not cover certain losses, such as:
ƒ Your own death or bodily injury due to motor accident
ƒ Your liability against claims from passengers in your vehicle
ƒ Loss, damage or liability arising from an act of nature i.e. flood, storm or landslide.

Note: This list is non-exhaustive. Please refer to the policy contract for the full list of exclusions under this policy.

7. Can I cancel my policy?


You may cancel the policy by giving written notice and your original Certificate of Insurance to us. Upon cancellation, any
refund of the premium would be based on the conditions stipulated in the policy contract.

8. What do I need to do if there are changes to my contact details?


It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a
timely manner.

Sila baca Lembaran Pendedahan produk ini sebelum anda membuat keputusan untuk membeli Insurans SmartDrive Kereta Persendirian.
Pastikan juga anda telah membaca terma-terma dan syarat-syarat am.

1. Apakah produk ini?


Polisi ini memberikan perlindungan ke atas liabiliti terhadap kecederaan atau kematian pihak lain, kerosakan harta pihak
lain, dan kerosakan akibat kebakaran atau kemalangan terhadap kenderaan anda atau kecurian terhadap kenderaan anda.

2. Apakah perlindungan / manfaat yang disediakan?


Manfaat polisi ini ialah :

A. Kerugian atau kerosakan pada kenderaan milik anda yang disebabkan oleh kebakaran akibat kebakaran, kemalangan
atau kecurian.
B. Pemandu yang anda benarkan untuk memandu kenderaan anda atau anda sebagai pemandu menangung liabiliti
seperti di bawah :
ƒ Kecederaan badan dan kematian pihak ketiga, dan
ƒ Kerugian atau kerosakan harta pihak ketiga,

Dengan membayar premium tambahan, anda boleh mendapatkan perlindungan manfaat seperti :
ƒ Kerosakan cermin depan
ƒ Perlindungan liabiliti penumpang
ƒ Rosak akibat dihempap oleh banjir, taufan, hurikan, ribut, ribut kencang, letusan gunung berapi, gempa bumi, tanah
runtuh, gelinciran tanah, penurunan atau penenggelaman tanah ataupun konvulsi semula jadi yang lain
ƒ Mogok, Rusuhan dan kekecohan Awam
ƒ Pampasan penilaian Pembaikan/Baikpulih
ƒ Kelayakan Diskaun Tanpa Tuntutan yang sedia ada
ƒ Aksesori kenderaaan

NOTA: Adalah satu kesalahan di bawah undang-undang Republik Singapura untuk memasuki negara tersebut tanpa
perlindungan liabiliti penumpang di dalam insurans kenderaan anda.

Tempoh perlindungan selama satu tahun. Anda dikehendaki memperbaharui polisi insurans anda setiap tahun.

3. Berapakah premium yang saya perlu bayar?


Jumlah bayaran premium yang perlu dibayar mungkin berubah mengikut kelayakan Diskaun Tanpa-Tuntutan (NCD), nilai
insurans kenderaan anda, keupayaan enjin dan keperluan pengunderaitan syarikat kami.

Sebagai contoh:
Jumlah yang diinsuranskan untuk kenderaan (baru) anda : RM40,000
Keupayaan Enjin : 1300 c.c
Perlindungan komprehensif : RM1,239.20
Kelayakan NCD : 25 peratus (RM309.80) Untuk jumlah sebenar, sila
Manfaat tambahan - Kerosakan cermin depan : RM90.00 rujuk sebut harga
Anggaran jumlah premium bersih yang perlu dibayar : RM1,019.40
(Tidak termasuk duti setem dan Cukai perkhidmatan)

* Jumlah yang diinsuranskan adalah berdasarkan nilai pasaran semasa kenderaan yang dirujuk kepada pengkalan data
sistem penilaian kenderaan Redbook.

4. Apakah yuran dan bayaran yang saya perlu bayar?


Apakah yang anda perlu bayar sebagai tambahan kepada premium Jumlah
Cukai perkhidmatan (jika ada) 6% daripada premium
Duti Setem RM10.00

Apa yang dimasukkan ke dalam premium Amount


Komisen dibayar kepada pengantara insurans (jika ada) 10% daripada premium 1 of 2
5. Apakah terma dan syarat utama yang saya harus ketahui?
Sebahagian terma-terma serta syarat-syarat utama yang perlu diberi perhatian adalah:
ƒ Tanggungjawab untuk memberitahu – Anda perlu memberitahu semua fakta berkenaan keadaan fizikal kenderaan
seperti sejarah kemalangan serta modifikasi ke atas keupayaan enjin.
ƒ Tunai sebelum Perlindungan - Adalah asas dan mutlak pada keadaan khas dalam perjanjian Polisi dimana premium
tamat tempoh mesti dibayar dan diterima oleh pihak kami sebelum tarikh mula perlindungan. Sekiranya syarat ini tidak
dipenuhi secara langsung polisi insurans ini terbatal.
ƒ Adalah penting anda memastikan kenderaan anda diinsuranskan pada jumlah yang betul untuk mengelakkan daripada
terkurang atau terlebih nilai yang diinsuranskan.
ƒ Lebihan Kekurangan Wajib -
a) Jika kenderaan anda dipandu oleh:
b) Seseorang yang berumur kurang dari 21 tahun;
c) Seseorang yang memegnag Lesen “L”;
d) Seseorang yang memegang lesen memandu penuh kurang dari 2 tahun;
e) Seseorang yang tidak dinamakan di dalam polisi
Jumlah kerugian yang perlu anda tanggung = minima RM400.00 (Lebihan kerugian wajib).
ƒ Jika berlaku kemalangan dan tuntutan akan ke atas polisi dibuat,
a. Anda dikehendaki membuat laporan polis dengan segera dan kami hendaklah diberitahu secara bertulis dengan
melampirkan semua butir-butir kejadian.
b. “Betterment” akan dikenakan jika kenderaan anda berumur lebih 5 tahun;
c. Anda mungkin tidak layak mendapat Diskaun tanpa Tuntutan semasa membaharui Polisi anda jika anda membuat
tuntutan ke atas polisi terhadap kenderaan anda.

Nota : Senarai ini bukannya senarai yang lengkap. Sila rujuk sampel kontrak polisi untuk senarai penuh bagi terma dan
syarat di bawah polisi ini.

6. Apakah pengecualian utama di bawah polisi ini?


Polisi ini tidak memberikan perlindungan terhadap kerugian tertentu, sebagai contoh:
ƒ Kematian anda sendiri atau kecederaan anggota badan disebabkan oleh kemalangan motor;
ƒ Liabiliti anda terhadap tuntutan daripada penumpang di dalam kenderaan anda; dan
ƒ Kerugian, kerosakan atau liabiliti akibat daripada bencana alam seperti banjir, ribut atau tanah runtuh.

Nota: Senarai ini bukannya senarai yang lengkap. Sila rujuk sampel kontrak polisi untuk senarai penuh bagi pengecualian
di bawah polisi ini.

7. Bolehkah saya batalkan polisi ini?


Anda boleh membatalkan polisi anda bila-bila masa dengan memberikan notis bertulis kepada syarikat kami. Anda harus
menyerahkan sijil insuransasal kepada kami.
Setelah pembatalan dilakukan, anda layak menerima bayaran balik premium berdasarkan syarat dan terma polisi anda.

8. Apakah yang harus saya lakukan jika terdapat perubahan dalam butiran peribadi saya?
Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi
memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya.

PERSONS OR CLASSES OF PERSONS ENTITLED TO DRIVE*
(a) The Insured
(b) Any other person who is driving on the Policyholder's order or with his permission

Provided that the person driving is permitted in accordance with the licensing or other laws or regulations to drive the Motor Vehicle or has been so
permitted and is not disqualified by order of a Court of Law or by reason of any enactment or regulation in that behalf from driving the Motor Vehicle.

LIMITATIONS AS TO USE*
Use only for social, domestic and pleasure purposes and for the Insured's business.
The policy does not cover use for hire or reward, racing, pace-making, reliability trial, speedtesting, the carriage of goods other than samples in
connection with any trade or business or use for any purpose in connection with the Motor Trade.

ORANG ATAU KELAS ORANG YANG LAYAK MEMANDU*


(a) Pihak Diinsuranskan
(b) Orang lain yang memandu di atas arahan Pemegang Polisi atau dengan kebenarannya

Dengan syarat orang tersebut dibenarkan memandu menurut undang-undang perlesenan atau undang-undang lain atau peraturan
untuk memandu Kenderaan Bermotor tersebut atau telah dibenarkan dan tidak dilucutkan kelayakannya di atas perintah Mahkamah
atau oleh sebab mana-mana enakmen atau peraturan daripada memandu Kenderaan Bermotor tersebut.

HAD PENGGUNAAN*
Digunakan untuk tujuan sosial, domestik dan persiaran dan berkaitan dengan perniagaan Pihak Diinsuranskan.
Polisi ini tidak melindungi kegunaan untuk sewaan atau ganjaran, perlumbaan, mengkadar kelajuan, ujian kebolehpercayaan, ujian
kelajuan untuk membawa barangan (selain dari sampel) berkaitan apa-apa pekerjaan atau perniagaan atau kegunaan untuk apa-apa
tujuan yang berkaitan dengan Perdagangan Motor.

CERTIFICATE OF INSURANCE
/ SIJIL INSURANS
Original / Asal

IMPORTANT : This Certificate is not transferable to a new owner of the vehicle

If for any reason this insurance is terminated during its currency, the Certificate must be returned to the Insurer, or if the
Certificate has been lost or destroyed a Statutory Declaration to that effect must be made. Cancellation of the Policy at the
request of the Insured will only take effect from the date of receipt by the Company of this Certificate or the Statutory
Declaration. Failure to comply with this obligation is an offence under the Compulsory Insurance Legislation.

The Certificate must be returned if the insurance is suspended during its currency.

IMPORTANT NOTICE : The attention of the Policyholder is drawn to conditions 2, 4 & 5 of this policy.

By reason of the Conditions referred to-

If the Policyholder or person driving the vehicle at the time of the accident:-
(1) (a) fails to give prompt notice to the Company in writing of the occurrence of the accident.
(b) fails to inform the Company of any writ summons or other notice of impending claim.

OR

Without prior advice to and receipt of written authority from the Company:-
(2) (a) admits responsibility for the accident.
(b) makes any offer promise or payment in respect thereof.

The Policyholder shall not be entitled to any benefit under this policy and the Company shall be entitled to be indemnified
by the Policyholder to the extent of any sum paid by the Company by virtue of Statute or otherwise.

PENTING :
Sijil ini tidak boleh dipindahmilik kepada pemilik baru kenderaan ini.

Sekiranya insurans ini ditamatkan semasa dalam tempoh perlindungan, Sijil hendaklah dikembalikan kepada Penginsurans,
atau sekiranya Sijil ini hilang atau musnah, pengisytiharan berkanun perlu dibuat. Pembatalan polisi atas kehendak Pihak
Diinsuranskan berkuatkuasa apabila Pihak Syarikat telah menerima Sijil atau Pengisytiharan Berkanun. Kegagalan
memenuhi tanggungjawab ini adalah satu kesalahan di bawah Perundangan Wajib Insurans.

Sijil hendaklah dikembalikan sekiranya insurans digantung buat sementara waktu semasa tempoh perlindungan.

NOTIS PENTING :
Sila ambil perhatian pada syarat 2,4 & 5 di dalam Polisi ini.

Syarat ini merujuk :


(1) (a) gagal memberi notis bertulis kepada Syarikat secepat mungkin mengenai kejadian yang berlaku.
(b) gagal memberitahu Syarikat mengenai sebarang surat dakwaan, saman atau notis tuntutan akan datang.

ATAU

Tanpa terlebih dahulu memaklumkan kepada dan menerima kuasa bertulis dari Syarikat:-
(2) (a) mengaku bertanggungjawab atas kemalangan tersebut.
(b) membuat sebarang tawaran, janji atau pembayaran.

Pemegang Polisi tidak berhak mendapat sebarang manfaat di bawah polisi ini dan Syarikat berhak mendapat gantirugi
dari pemegang polisi atas sebarang jumlah yang dibayar oleh Syarikat disebabkan

NOTICE / PEMBERITAHUAN :
For all the intents and purposes where there is a conflict or ambiguity as to the meaning in the Bahasa Malaysia provisions
of any part of the Contract, it is hereby agreed that the English version of the Contract shall prevail.
