    AFFIDAVIT OF UNDERTAKING

    I, JUAN R. ALVAREZ, of legal age, married, Filipino and a resident of


    Brgy. Lanjagan, Ajuy, Iloilo after having been sworn to in accordance with law
    hereby depose and say:

    1. I was the owner and holder of a Firearm described as follows:

    Kind : Pistol
    Make/Model : Remington
    Caliber : .45 pistol
    Serial Number : 883832

    2. That I kept the said firearm inside my stock room in my residence in


    Ajuy, Iloilo;

    3. That last October 22, 2007, while I was cleaning my stock room, I
    discovered that the said firearm was lost and I could no longer locate the same
    despite diligent efforts. Upon discovering that the said firearm was lost, I
    immediately reported the same to the Ajuy Municipal Police Station. A copy of the
    Certification issued by the Ajuy Municipal Police Station is hereto attached as
    Annex “A”;

    4. While the said firearm was still in my possession it was never used in the
    commission of any crime or any unlawful act; and

    5. I am executing this affidavit of undertaking to attest to the truth of the


    foregoing.
