Step 2. Pay for two pcs documentary stamps (one stamp on this form and one for the Certificates
of Registration at the Customer Service Center.
Step 3. Present duly accomplished oath form for verification of records and assessment at Window
12.
Step 6. Sign in the Registry Sheet and receive your claim slip at:

REQUIREMENTS
New registrants should come personally to the PRC Central Office or to the nearest Regional Office
to file his/her application for registration and to affix his/her signature on the registry book.
Applicant for registration under the Special Professional Licensure Board Examination and those
who left for abroad immediately after their examinations or before the results of the examinations
are released shall:
1. Take his/her professional oath before the Philippine Consul.
2. Submit a photocopy of his/her passport pages showing its personal details, date of entry and
departure from the Philippines.
3. Submit a Special Power of Attorney notarized by the Philippine Consul.
Certificates of Registration shall be claimed at the Central Office or the Regional Office where the
application for the same has been filed. Representatives must present Special Power of Attorney
and appropriate identification papers.
