    BSO/WH-ER-ASN/DGR/DANKUNI/KOLKATA WITHOUT ANY TRANSFER BENEFITS ON THE
    BASIS OF COMPASSIONATE HEALTH GROUNDS OF MY FAMILY & ALSO ON THE BASIS OF
    DEPARTMENTAL SERVICE QUOTA VACANCY-REG.
    At the very outset, kindly excuse me for taking your precious time to read my
    request in your busy schedule. This is to officially express my intent to be transferred to
    CMO at Eastern Region (ASN/ DGR/KOLKATA).
    I am writing to respectfully request a transfer in location from Hyderabad to Kolkata.
    A change in my personal circumstances necessitates this move. A major illness in my
    personal life and entire family made it acute necessary for me to relocate to my native
    area of any of the above choices on departmental service quota vacancy. Since I have
    been in organisation for 1year 2months, and have thoroughly enjoyed my employment
    here, I feel it is in the best interests of myself and of this company that I move my native
    area, Rather than leave the company altogether due to family problems.
    I would like to bring to your kind notice that I am working as Jr.Assistant bearing
    P.No:-G-101883 in SAIL-CMO-WH-SR-HYDERABAD W.E.F 10-02-2016.
    With due respect and humble submission I would like to lay down the following few
    lines for your Kind consideration.
    1. My mother is suffering from HYPERTENSION WITH SEVERE OSTEOARTHRITIS
    of both Knee Joints along with LUMBAR SPONDYLOSIS, often associated with
    constipation, stress, depression, anxiety. Frequent medical attention is required. After
    my Father’s sudden demise my mother is alone. There is no one to look after her at
    home. It is tough for her to undergo repeated medical assistance singlehandedly. My
    father’s death certificate is herewith attached with this application.
    2. My Father, Vidya Sagar Jaiswal is an Employee of SAIL-ISP, Burnpur has done service of over
    32years. My Father untimely demise is due to Gastric Ulcer and breathed his last on 13-10-2013.
    In the view of above, I appeal to your good self to consider my case sympathetically
    and permit me to have early transfer to SAIL-CMO-BSO-ER-ASN/DGR/KOLKATA so that I
    can deliver my personal and official commitments in better way. West Bengal is my Native
    state and it would be easy for me to help my mother to have medical treatment & regular
    checkups at any of the areas of my preferred choices easily.
    The following list of Supportive and Required Documents Xerox copies’ are Herewith enclosed for your
    Kind consideration.
    1. Mother Medical Reports.
    2. Father Death certificate & Proof of service certificate in SAIL-ISP, Burnpur.
    3. Service Identity Card
