In June 1999, Jerri Nielsen did a routine breast self-examination and discovered a lump. As a
physician, she understood immediately that she was about to become a patient – one with a big
problem, since she was also the only doctor at the South Pole, in winter. Trapped there for at least five
more months by temperatures averaging minus 80º F, Nielsen knew she couldn’t afford to wait to find
out whether the lump was a malignant tumour, requiring immediate treatment.
Unlike past explorers, who ventured to the ends of the Earth, Nielsen and her colleagues at
Amundsen- Scott Station had a lifeline to the rest of the world – a satellite telephone and Internet
connection.
After performing a biopsy on herself, she was able to capture digital images of the tumour
cells, send them to pathologists in the United States for analysis, and then discuss her case by
videoconference with oncologists at the National Cancer Institute.
With their guidance - and an airdrop of chemotherapy drugs – Nielsen was able to start
treating herself, even though she’s a primary care physician, not a cancer specialist. Then she was
able to hang on and wait for a break in the weather, knowing she wasn’t losing precious time in her
fight against this life-threatening condition.
Dr. Nielsen couldn’t perform surgery on herself in Antarctica. But there had been another
doctor on hand with even a little surgical experience, surgery may very well have been an option with
remote coaching by an expert.
