35 IV. OPENING STATEMENT: Dr. Dominic R. Basalo
36 Municipal Health Officer of RHU Carmen and former Cluster President
37 acknowledged the presence of Interlocal Health Board Members of Cluster 3 Davao
38 del Norte highlighting that the present meeting will be the first for year 2018 and that
39 the support and attendance of Local Chief Executives signifies solidarity in the
40 cluster.
41 There was a motion from Dr. Dahlia Cañete of the Provincial Health Office to include
42 the Provincial Program Implementation Review (PIR) and PHO Inter-local Health
43 Budget 2018 in the agenda seconded by Dr. Dominic R. Basalo.
44 Adoption of agenda was moved by Ms. Rebecca Dollete, RN and seconded by Dr.
45 June P. Lim.

47 V. PRESENTATION OF AGENDA
48 Dr. Juri Miyake presented the Agenda of the First Cluster 3 (DCaPS) Inter-local
49 Health Board Meeting, to wit:

58 1. 2017 Cluster 3 Accomplishment Report

59 Ms. Rebecca Dollete, RN presented the over-all accomplishment of the Inter-local
60 Health Board for the previous year 2017. She mentioned that the board has
61 conducted a total of eleven (11) Inter-local Health Board Meetings for the whole
62 year 2017 which is beyond the targeted number of meetings annually. She also
63 added that there are still seven (7) Resolutions pending for the approval of Cluster
64 President. She further presented in detail that within the cluster, there were a total
65 of twenty-one (21) Local Health Board Meetings conducted in four (4) Local
66 Government Units: 3 meetings in Panabo City, 5 meetings in Sto. Tomas, 6
67 meetings in Carmen, and 7 meetings conducted in B.E. Dujali. Moreover, out of
68 these twenty-one (21) local health board meetings, there were a total of nineteen
69 (19) resolutions passed and five (5) ordinances formulated. Panabo City had two
70 (2) resolutions passed, six (6) by Sto. Tomas, and eight (8) by Carmen, and three
71 (3) resolutions passed by B.E. Dujali. Two (2) ordinances were formulated by Sto.
72 Tomas and three (3) ordinances by B.E. Dujali.
73 Ms. Rebecca Dollete, RN requested the members of the board to come-up with
74 more resolutions for the present year.
75 In line with the pending and targeted resolutions by the cluster, Dr. Dominic R.
76 Basalo, took the opportunity to update on the Cluster 3 (DCaPS) Referral Manual.
77 He reiterated to the board the importance of coming-up with a written document
78 on the policies of the cluster service delivery network to enhance the validity and
79 the applicability of the existing Manual of Referral. This is in response to the
80 involvement of the Manual of Referral as a requirement in the renewal of the
81 Primary Care Benefit of the Local Government Units.
82 The board has decided to formulate an Inter-local Health Board Certification of
83 the existing Manual of Referral, and to update and/or enhance the commitment of
84 the Local Chief Executives to the abovementioned Manual of Referral.
85 2. Cluster 3 Surgical Caravan
86 Dr. Dominic R. Basalo opened the agenda by mentioning that in the previous
87 year, two (2) surgical caravans were conducted in Kapalong and Samal, and for
88 the present year, the Department of Health (DOH) decided to conduct the surgical
89 caravan in Carmen, Davao del Norte covering surgical clients from the whole

 90 Cluster 3. The Surgical Caravan will be funded by the DOH utilizing funds from
91 the Medical Assistance Program (MAP) Funds; and with manpower from the
92 Davao Regional Medical Center (DRMC). The role of the Local Government
93 Units under Cluster 3 is to identify patients per municipality who are eligible to
94 avail the services which will be offered during the surgical caravan. Davao del
95 Norte Hospital Carmen Zone will also serve as the main surgical facility where
96 procedures will be performed.
97 A motion was made by Dr. Juri Miyake for the cluster to assign Point Persons per
98 municipality who will be responsible in facilitating the screening, briefing, and
99 assisting surgical clients in peri-operative and post-operative transactions and
100 management: this is to prevent disorder in patient management during the whole
101 activity. This motion was seconded by Dr. Dominic R. Basalo and the rest of the
102 board.
103 The tentative date for the Surgical Caravan is scheduled on June 4 to June 8,
104 2018. In order for this date to be finalized, a resolution should me made
105 designating the aforementioned date as the schedule of the Surgical Caravan in
106 Cluster 3 Davao del Norte.
107 Dr. Dominic R. Basalo also emphasized that this agenda should be included as an
108 agenda on the next scheduled meeting in order to guarantee adequate preparation
109 for the surgical caravan.
110 A clarification was made by Dr. June P. Lim addressed to all members of the
111 board if it is possible for surgical patients registered to avail the surgical services
112 from the caravan within the set schedule can still avail or undergo the surgical
113 procedure even if the operation will be scheduled on a date beyond the 4 th and 8th
114 day of June 2018. Dr. Dominic R. Basalo responded that patients can still be
115 operated beyond the set schedule of the surgical caravan provided that they are
116 already registered to avail services from the surgical caravan within the set
117 schedule. This response was seconded by Dr. Juri Miyake.
118 3. Cluster 3 (DCaPS) Referral Updates
119 Dr. Dominic R. Basalo reiterated that per existing Cluster 3 Manual of Referral,
120 hospitals must gather all return slips and a point person per Municipality shall

121 collect these return slips. This policy was also recapped by Ms. Rebecca Dollete,
122 RN, requesting Davao Del Norte Hospital Carmen Zone to keep all referral forms
123 and wait for the point persons assigned by respective municipalities to collect
124 them. She also suggested that every municipality should assign a point person
125 who will be responsible in collecting return slips from other private facilities
126 where patients may have been referred to and received needed health services.
127 This is to make the Two-Way Referral system of the cluster more comprehensive
128 not focusing only on referrals made to government health facilities. It was also
129 decided by the board that an endorsement letter assigning HRH/Personnel to
130 collect return slips shall be made and sent to various health facilities within the
131 cluster.
132 She also presented a system to strengthen the implementation of the Two-Way
133 Referral system in the Barangay Health Station (BHS) and Rural Health Unit
134 (RHU) level: she mentioned that all BHS Nurse/Midwife (LGU Hired or HRH)
135 must provide one (1) envelope to the RHU to serve as a Filing Envelope where
136 return slips will be kept and organized per Barangay, and the return slips kept in
137 these envelopes will be collected again by BHS personnel weekly for recording
138 and documentation.
139 Furthermore, a request was raised by one member of the board for the cluster to
140 also formulate an Inter-local Health Zone Disaster Plan aside from the Manual of
141 Referral which was already developed by the cluster. Dr. Dominic R. Basalo
142 immediately responded that this suggestion is indeed feasible and would
143 maximize the potential of the cluster in helping one another in times of disasters
144 and other public health events. He also said that the boarded may start taking
145 steps on this matter by first making a resolution that would request the Provincial
146 Government to assist the Cluster 3 Davao del Norte in the formulation of Cluster
147 Disaster Plan.
148 4. Feed-backing Survery Form
149 Ms. Rebecca Dollete, RN reminded all MHOs and other RHU personnel that
150 findings obtained from Feedback Survey forms must be relayed to all Local Chief
151 Executives. The rationale of this feed-backing survey form is for the municipal

152 local health board members to identify the problems existing in the delivery of
153 health services to the municipal and barangay level and to acknowledge
154 recommendations from clients in enhancing the delivery of health services. The
155 local health board members will also be able to determine ways to improve the
156 present practice and establish an action plan.
157 Dr. Juri Miyake also suggested that Suggestion Boxes must be opened and
158 consolidated weekly by respective facilities. This suggestion was seconded by
159 Ms. Rebecca Dollete, RN stating that survey forms from BHS should also be
160 dropped in the Suggestion Box placed in the RHU weekly for consolidation and
161 documentation. She also added that Suggestion Boxes should be kept locked and
162 be opened only by assigned personnel.
163 As to the supply and availability of Survey forms to be distributed and utilized by
164 many health facilities within the cluster, Dr. Dominic R. Basalo requested the
165 Provincial Health Office to assist the cluster in the reproduction of forms.
166 In response, Mr. Dominador Jadraque of the Provincial Health office Davao del
167 Norte reassured the members of the board that the PHO can offer their Risograph
168 Printing machine should there be a need for the cluster to duplicate survey forms
169 provided that the cluster will shoulder the supply of paper rims.
170 6. Mayor Leah Marie Moral-Romano’s Request (Medical Mission)
171 Hon. Leah Marie Moral-Romano, the Local Chief Executive of the Municipality
172 of Braulio E. Dujali presented to the members of the board the proposal for the
173 cluster to accommodate and assist a Week-long Medical Mission by the
174 Philippine Nurses Association of America (PNAA) which has a target schedule
175 on January 21-24, 2019. Hon. Lean Moral-Romano mentioned that the Medical
176 Mission will also offer dental and surgical services. The providing organization
177 will bring their own supply of medications and health equipment which they will
178 use during the whole course of the medical mission. The organization will
179 conduct the medical mission in all four (4) municipalities under the DCaPS
180 Cluster in separate dates. However, all surgical procedures will only be offered
181 and performed in the Davao del Norte Hospital Carmen Zone; hence a
182 clarification was directly addressed to the Chief of Hospital Dr. Juri Miyake if this

183 surgical set-up is applicable and allowable, to which Dr. Juri Miyake responded
184 positively provided that a close collaboration accompanied by a formal letter
185 and/or request will be made ahead of time.
186 Moreover, Hon. Leah Marie Moral-Romano also requested the support of other
187 Local Chief Executives from Sto. Tomas, Carmen, and Panabo, as an LGU
188 Counterpart, to shoulder expenses on the Food (Lunch, Snacks, and Dinner only)
189 of the foreign health team members, as well as their transportation from their
190 hotel in Davao City to the municipalities and vice versa. She further specified that
191 LGU-Sto. Tomas and LGU-Carmen can offer their Municipal Buses as the main
192 transportation vehicle for the whole team. In addition, she also requested the LGU
193 to prepare and fund a Welcome Dinner and a Sent-Off Dinner for the entire
194 Medical Mission Team.

195 To add more details on this agenda, Dr. Cherry T. Demaala of the RHU Carmen
196 also elaborated on the proposal made by Hon. Leah Moral-Romano. Dr. Demaale
197 mentioned that more than the accommodation of the medical mission team, what
198 is more important is the Master-listing of Surgical Patients before the medical
199 Mission schedule. She asked for the support and assistance of the Municipal/City
200 Health Officers of the respective LGUs to act as Primary Personnel in developing
201 a master-listing ahead of time. She also added that a Cardio-Pulmonary Clearance
202 will be required from all surgical patients and the expenses will no longer be
203 covered by the Medical Mission Team. Thus, a verbal request was addressed to
204 the Representatives of Board Member Roy Catalan, Ms. Sherly L. Rosal and Ms.
205 Edlyn N. Tabanicra if the cluster can receive support and funding from the
206 Congressional Fund.

207 Dr. Cherry T. Dimaala also added that the Cluster is also requested to shoulder the
208 expense for the Medical Mission Tarpaulin with a template coming from the
209 Medical Mission Team.

217 Dr. Cherry T. Demaala ended her presentation on the agenda by requesting the
218 members of the board to include this matter in all succeeding board meetings for
219 updates.

221 Other Matters:
222 A. Provincial Program Implementation Review (PIR)
223 Dr. Dahlia Cañete of the Provincial Health Office presented to the members of the board
224 the set date for the coming Program Implementation Review which is scheduled on
225 February 28-March 2, 2018. Venue of the PIR is still to be finalized; the said PIR will be
226 participated by all Municipal/City Health officers and Public Health Nurse. The main tool
227 that will be used during the PIR will be the LGU Scorecard (with 26 indicators) and
228 Performance/Accomplishment on other actual programs (e.g. Oral Health Program).

230 B. PHO Inter-local Health Budget 2018.

231 Dr. Dahlia Cañete reiterated to the members of the board that the PHO will fund four (4)
232 Inter-local health zone meetings annually. She also presented that the Province will be
233 conducting a Health Summit on May 2018 where the Awarding of Top Performing
234 LGU’s will be incorporated. The basis for rating/judging LGUs will be the LGU
235 Scorecard. In relation to the said awarding, the Governor, Hon. Anthony G. Del Rosario,
236 funded cash prizes as stated below:

237 1st Top Performing LGU – P 500,000.00

238 2nd Top Performing LGU – P 300,000.00

239 3rd Top Performing LGU – P 100,000.00

240 A concern however was raised by the members of the board as to the validity and
241 reliability of the rating and judging per LGU. This concern was immediately responded
242 by Mr. Dominador Jadraque stating that for this year’s awarding, an Evaluation and

243 Screening Committee will be formed in order to come up with a more transparent system
244 of judging.

245 C. Hazardous Waste Disposal and Management

246 Dr. June P. Lim of RHU-Sto. Tomas presented to the members of the board
247 the existing waste concern on the municipality with regards to the
248 management and disposal of hazardous wastes after waste vaults are filled and
249 safety boxes are all kept and left unmanaged within the RHU.
250 In response, Dr. Dominic R. Basalo recommended to the board the
251 formulation of a resolution that will request for the presence of Engineer Raut
252 and a representative from the Department of Environment and Natural
253 Resources (DENR-EMB) to address existing concerns regarding hazardous
254 waste disposal.

255 Announcement
256 Dr. Dominic R. Basalo made an announcement on the schedule of the next Inter-
257 local Health Board Meeting. It was decided by the board that the succeeding
258 board meetings will be conducted every second Thursday of the month, hence
259 March 8, 2018 will be the set schedule for the next meeting.
