Classification Annual Survey Checklist
For annual or intermediate survey mark ‘X’ to indicate ‘yes’ and ‘O’ to indicate an outstanding action in appropriate block for
each item. If an
item is not applicable mark ‘N/A’ in appropriate block. All required dates should be recorded at these surveys.
Annual survey: to be held within 3 months before or after the anniversary date

6.1 Cargo tank openings including gaskets, covers, coamings, P/V valves and flame screens are all satisfactory as __

6.4 The condition of electrical equipment in dangerous zones is satisfactory as far as could be ascertained. __
Cargo pump room
6.5 Confirm that no potential sources of ignition (in or near the cargo pump room) such as loose gear, excessive __
product in bilges, excessive vapours, combustible materials, etc. have been observed and if found then
eliminated.
6.11 The cargo, bilge, ballast and stripping pumps examined as far as practicable and found satisfactory for:
(a excessive gland seal leakage. __

(b operation of electrical and mechanical remote operating and shutdown devices. __

(c integrity of pump room bilge system and pump foundations. __

6.12 The pump room ventilation system is operational, and the ducting intact, the dampers are operable and the __

6.17 The deck water seal filling and draining system is operational and without evidence of water carry-over. __
6.19 The operation of all remotely operated or automatically controlled valves, in particular the flue gas isolating __
valve(s) found satisfactory.
6.20 The interlocking feature of the soot blowers checked and found satisfactory. __
6.21 The gas pressure regulating valve automatically closes when the inert gas blowers are secured __
7. Docking Surveys
7.1 A satisfactory examination of the shell including bottom, side and bow plating, sea inlet boxes, keel, stern,
sternframe and
rudder(s) was carried out. __

6.24) are only required
to be dealt with on ships to which Part 6, Chapter 4 of the Rules for Ships applies.
3 At annual or intermediate survey where the periodical surveys referred to in 1.2, 1.3, 1.4 and 1.5 (see page 1) are due
or overdue these
are to be satisfactorily completed in order that the Certificate of Class can be endorsed. However where overdue
periodical surveys
cannot be completed at the time of annual/intermediate surveys as a result of extenuating circumstances then the
following procedure
may be adopted:
3.1 During attendance for all classification surveys the surveyor should review the classification status and establish that all
other due or
overdue Periodical Surveys have been dealt with or have been postponed by agreement. Circumstances may exist
when it is not
practical for the owners to present items for survey (e.g. port authorities do not always allow immobilisation) and, under
these or
similar circumstances, surveyors should undertake a general examination and testing/review as required, after
obtaining confirmation
from the Master or Chief Engineer that the item remains efficient, and agree to a postponement of survey for a period
not exceeding
three months from the due date of survey.
3.2 The items which may be postponed in these circumstances without reference to the committee are:
CSM and CSH items (other than cargo holds and SW ------------
ballast tanks)
CSM and CSH items should not be postponed beyond the completion date of the cycle without reference to London
Classification Section. When a postponement has been agreed the items should be included on the Interim
Certificate under the heading ‘Postponement of Survey’ and listed with the report narrative. The period of
postponement is to be
stated.
3.3 Owners request for postponement of surveys other than the above are to be London Classification Services or
referred to Regional Devolved Classification
Executive.
4 The Regulations Part 1, Chapter 2, Section 1.1.9 require that the International Convention Certificates, listed in Section
2.1, are issued by
Lloyd’s Register, a National Administration or an IACS Member when so authorised.
London Classification Services should be advised when this Regulation is not complied with.
5 For the application of the requirements outlined in the Regulations Part 1, Chapter 3, sections 2, 3, 4 and 5, a general
dry cargo ship is defined as a self propelled ship of 500 gross tonnes or above, constructed generally with a tween deck
and intended to carry solid cargos. This excludes bulk carriers, refrigerated cargo ships, roll on-roll off ships and ships
dedicated for the carriage of containers, forest products (but not log or timber carriers), wood chips and cement.

16 Has this ship a special feature class notation "LA" with respect to bow doors, stern doors or side __
doors used as cargo ramps for vehicle loading and discharging operations?
17 Has this ship an optional descriptive notation (CR) in column 6 of the Register of ships with __
respect to bow doors, stern doors or side doors used as cargo ramps for vehicle loading and
discharging operations?
18 If yes for 16 or 17, are the surveys required by the Cargo Gear Register up to date? __
*For all inward opening doors (without size restriction) and outward opening doors with a clear opening of 12m² or
more, this is a Rule requirement.
