as evidenced by Lab Results PLANNING After 6 hrs. of Nursing Intervention, the client will be able to identify individual risks and engage in appropriate behaviors or lifestyle changes to prevent bleeding INTERVENTION 1. Assess patient for any risk of bleeding 2. Monitor V/S 3. Monitor for Occult or Frank bleeding 4. Obtain Laboratory Tests (CBC) 5. Explain the different risks for bleeding 6. Explain the different factors to prevent bleeding 7. Restrain patient from any activities that could cause bleeding 8. Advise patient to use a soft bristle toothbrush 9. Administer appropriate Medications prescribed by the Physician EVALUATION After 6 hrs. of Nursing Intervention, the client was able to identify individual risks and engage in appropriate behaviors or lifestyle changes to prevent bleeding

Objective: - IVF Plain NSS 0.9% at Left Hand regulating at 28 gtts/min, No bleeding, no swelling - Platelet count of 66 seen in recent Lab Results
