This is very concerning. However, it is amendable to corrective action. In fact, one in ten (10%) events reported in this analysis were related to transitions of care and medication reconciliation. This is a critical time in a patient's therapeutic course. Conducting a thorough medication reconciliation is essential to avoid these types of errors.
