🟢 “The real breakthrough came in 2010, when a group of clinicians came together to form an emergency laparotomy network audit, which was part of the National Health Service (NHS) Networks." They prospectively collected data from 37 hospitals over 3 months in 2011 and identified an overall mortality rate of 14.9%, with an extraordinary mortality range of 3.6-41.7%. They also identified a wide variation in the standard of care of emergency laparotomy surgical patients, particularly in terms of resuscitation and early access to theatres, ICU admission and the input of consultant anaesthetists and surgeons. From a personal point of view, understanding that the mortality rate of patients undergoing emergency laparotomy at my hospital - Royal Surrey County Hospital - was 22%, was a real wake-up call and a major driver for me to become personally involved in improving the care of emergency laparotomy patients. One of my favorite professional writings was the following published in Critical Care Explorations; Automated Near Real-Time Ventilator Data Feedback Reduces Incidence of Ventilator-Associated Events: A Retrospective Observational Study Critical care teams play a vital role in improving patient outcomes by following best practice protocols. At Candler Hospital’s Medical-Surgical ICU, a retrospective review of clinical data over 24 months focused on weaning mechanically ventilated patients from ventilators to reduce ventilator-associated events. The study highlighted that compliance with protocols in most units is suboptimal. To address this, the implementation of the Respiratory Knowledge Portal showed promise. During the 11-month intervention phase, data presented on tablet computers during multidisciplinary rounds led to positive outcomes. Providers monitoring alerts for possible ventilator-associated events and guiding patients through spontaneous breathing trials are crucial steps in enhancing patient care. A direction of New Zealand. “The expansion of NHS surgical hubs in England should be “turbocharged,” as new research indicates that they can play a big role in cutting waiting lists, the Royal College of Surgeons of England has said.So far more than 100 NHS trusts in England have set up surgical hubs with ringfenced capacity to carry out operations such as hip replacements and cataract removals, aiming to improve productivity. New hubs were created to help healthcare services recover from the covid pandemic, with an initial focus on high volume low complexity (HVLC) specialties, and 26 more are planned to open by the end of 2025. The Health Foundation this week presented an analysis saying that surgical hubs could significantly increase treatment volumes, by around 11%.
