Seeing patients navigate a maze of uncoordinated care among different providers is both disheartening and alarming. This disjointed care journey leads to inefficiencies, repetitive procedures, and troubling gaps in treatment. The consequences of overlooking this are dire, with wasted resources and, crucially, compromised care quality leading to patient dissatisfaction. To address this, we can: ⦿ Embrace Integrated Care Models: A shift towards a collaborative, team-based care approach promises a unified patient experience, bridging the gaps in treatment. ⦿ Leverage Health Information Exchanges (HIEs): By promoting the use of HIEs, we can ensure secure patient information sharing among providers, enhancing coordination, reducing test duplication, and making informed care decisions. ⦿ Introduce Care Coordinators: These navigators ensure patients' healthcare journeys are coherent and streamlined, connecting each phase of care. ⦿ End-of-Life Care Planning: Stress the importance of an advanced directive and living will to enhance dignity and well-being for patients and their families. A clear end-of-life plan based on the patient’s wishes reduces family stress and prevents unnecessary, painful medical interventions. Embracing these strategies can transform our healthcare system into one that is more connected, efficient, and focused on delivering quality patient care. 5 lessons I didn’t expect on how health systems can successfully engage patients with Digital Health: 1️⃣ Clinician buy-in is a prerequisite to patient buy-in. The more clinicians are bought-in to a digital solution, the more strongly they promote it to patients and the higher patient engagement will be. No clinician adoption = no patient adoption. 2️⃣ Vendors must be accepted as full service partners, not simply as tech suppliers. When we’ve been told to simply provide our technology and get out of the way.... When our implementation playbook and change management best practices were ignored… The results were never good. When health systems leaned on our 10+ years of expertise and welcomed us with a seat at the table… The results have been far better. 3️⃣ Optimism and self-belief are critical ingredients to achieve the best outcomes. Our health systems with the highest patient adoption assume the digital health initiative will be successful. They assume patient readiness by default. The solution is opt-out, not opt-in. They tell patients they will be successful using it. Our health systems with the lowest patient adoption are hesitant to believe it will work. They assume patients will reject the solution by default. The solution is opt-in, not opt-out. They see elderly patients and don’t even offer the digital solution. 4️⃣ Evidence-based, clinical pathways are commoditized. The magic doesn’t come from some proprietary clinical knowledge. Best practices are widely known now. The magic (and value) comes from figuring out how tech can be used to increase patient adherence to known pathways. What’s the right messaging? Timing? Modality? There’s a science to engaging patients digitally, and it’s not found in any clinical guidelines. 5️⃣ No two organizations are alike. Change management plans must be tailored to each situation. Yes, patterns emerge and there may be similarities. But each organization has its own unique culture, patient demographics and digital readiness.
