📣 Calling on All UK Professionals/Residents to Stand Up for FGM Survivors! As an FGM survivor, I have faced physical and emotional pain that no one should endure, but my journey has only fueled my mission: to ensure that the UK government and NHS recognise FGM survivors and provide us the basic medical rights we deserve. Today, I’m urging you all, especially UK residents, to sign and share my petition for meaningful change. For too long, FGM survivors have heard excuses: “There isn’t enough research,” “It’s not beneficial,” or even “It’s unnecessary.” But how can judgments be made by those who have no experience with our trauma, our pain? If Germany, France, Spain, and even Egypt can offer reconstruction surgeries to FGM survivors, then why not the UK? My surgery in Germany left me in debt, yet I pursued it because the UK’s failure to provide this care is intolerable. No one should be told they “don’t matter”—especially not after surviving one of the worst forms of violence. FGM survivors are mothers, daughters, professionals, and advocates. We demand the same medical options others receive. When we’re denied these rights, it’s a blatant disregard for our dignity and health. During a recent cesarean section at our Maternity Hospital, I encountered a new case for me —a patient with pronounced abdominal muscle weakness and a concurrent ventral hernia. This prompted me to discuss with my supervisor the feasibility of performing both surgeries simultaneously. Upon deeper inquiry, I realized cesarean sections are classified as clean-contaminated procedures, carrying a heightened risk of surgical site infections (SSI) if prosthetics are involved. As a healthcare professional with a keen interest in health economics, this raised questions about the principles of overlapping and simultaneous surgeries. In my exploration, I've come to appreciate that hospital policies and practices must prioritize patient safety and adhere to evidence-based medicine. Efficient management of surgeries like cesarean sections and hernia repairs can yield substantial benefits, but always within the framework of rigorous safety protocols. Prior literature suggests standard enoxaparin dosing may not achieve target prophylactic anti-Xa levels. We hypothesize that a new weight-based enoxaparin protocol with anti-Xa monitoring for dose titration in critically injured patients is safe and easily implemented. Methods This prospective observational study included patients with trauma admitted to the trauma intensive care unit (ICU) from January 2021 to September 2022. Enoxaparin dosing was adjusted based on anti-Xa levels as standard of care via a performance improvement initiative. The primary outcome was the proportion of subtarget anti-Xa levels (<0.2 IU/mL) on 30 mg two times per day dosing of enoxaparin. Secondary outcomes included the dosing modifications to attain goal anti-Xa levels, VTE and bleeding events, and hospital and ICU lengths of stay. Conclusion A majority of critically injured patients do not meet target anti-Xa levels with 30 mg two times per day enoxaparin dosing. This study highlights the need for anti-Xa-based dose modification and efficacy of a pharmacy-driven protocol. Further optimization is warranted to mitigate VTE events.
