Pin and Clamp Constructs for Paediatric Proximal Humerus Fractures 🛠 Dear colleagues and seniors, I am reaching out to tap into the collective wisdom of the global orthopaedic community regarding the use of pin and clamp constructs for managing paediatric and adolescent proximal humerus fractures. In my recent cases, I've employed this method and found it valuable for achieving stable fixation and promoting fracture healing. 2 patients were less than 13 years old, and one was 17. And all of them demonstrated excellent outcomes. However, I'm eager to learn more about your experiences, techniques, and outcomes with this approach. 1. Interesting case for the LI community. 65-yo-male with multiple previous lumbar procedures, with L3 to ilium instrumentation and SCS. Presents with significant debilitating axial LBP, bilateral radicular leg pain and difficulty standing upright. He also had a painful, nonfunctional SCS. Imaging showed, - L5-S1 pseudarthrosis - Distal junctional failure with loosening of pelvic screw - Sagittal imbalance - L2-3 adjacent segment degeneration with instability We offered him a staged revision. Surgical objectives included establishing harmonious sagittal alignment, indirect decompression of L5-S1 neuroforamina and stabilizing L2-3 and L5-S1. There are roughly 150,000 fragility fractures of the pelvis (FFP) per year in the US. Many patients today are managed conservatively with bed rest, pain medication, and mobility assistance.
