One common misconception is that merely opening the switch meets the requirement of system de-energizing. However, a system should not be considered properly de-energized until all the hazardous energy control requirements are met properly, as defined by OSHA 1910.269(m) for de-energizing and OSHA 1910.269(n) for grounding requirements (Occupational Safety and Health Standards – Title: Electric power generation, transmission, and distribution.). Safety takes the highest priority in any electrical works. Safe operations and safer workplaces are the prerequisites for minimal interruptions in the power supply. In the same way, uninterrupted power supply has gained such importance that the consumers spent a substantial sum on backup power and power-switching equipment to ensure the continuous flow of power.

When an operator manually charges a circuit breaker, several potential faults could arise that might compromise its operation. For instance, incomplete charging can occur if the handle isn’t fully engaged, leading to a situation where the breaker doesn’t close properly. Over-tensioning the mechanism could damage internal components, while mechanical jamming might result from dust or debris obstructing the parts. Additionally, worn or damaged springs could be exposed during the manual process, revealing weaknesses that affect the breaker's reliability. There’s also the risk of the mechanism failing to latch correctly, or not resetting as it should after charging. Considering these possibilities, what other faults could potentially occur as shown in this video??
