Menarini Stemline is a prime example of the use of precision medicine in the service of patients. They are pioneers in the approval of companion diagnostics (CDx) for targeted drugs in particular for breast cancer. Their product Elacestrant, as an oral SERD, has been approved for ESR1 mutated cases with a specific prior treatment profile, and Guardant360 CDx is the approved diagnostic test. **But here is the thing...** some patients whose test came back without a definitive result, and some patients who test negative for ESR1 mutations, ALSO stand to benefit from this oral SERD. Should it be up to the patients? Should they have a say on whether to give this medication a try? Here are a few important considerations: 1. Although driven by ESR1 mutated patients, PFS in ALL patients in the EMERALD trial showed improvement with Elacestrant without any sign of a negative impact on overall survival. 2. Oncologists routinely prescribe treatments for off-label use, taking into account their experience and their knowledge over the intricacies of their patient's case. 3. Only the patients themselves are in position to know how any benefit-risk ratio truly articulates within the specificity of their own lives 4. Wider use of both medications and testing will generate real-world data that will allow for ever more precise definitions of WHICH patients do stand to benefit. It is time we empower patients over their own testing data and health and treatment decision-making. We need to develop regulatory pipelines that will allow for this as it will unlock much needed development of precision medicine through real-world data, and at a scale that cannot be matched through piecemeal clinical trials. I think a good first step will be collecting data on the patient's point of view and experience with both companion diagnostic testing and the treatment decision-making process, in particular as they relate to all of the new approvals since 2020. Recognizing the Importance of Early Detection: With one in eight women expected to develop breast cancer during their lifetime, per the CDC, early detection is the key to saving lives. When discovered early, and in the localized stage, the 5-year relative survival rate for breast cancer is 99%, per the American Cancer Society. The most important screening test for breast cancer is a mammogram, an X-ray of the breast that can detect breast cancer up to two years before the tumor can be felt by you or your doctor. “Patients should start anywhere between the ages of 40 and 50 and then get mammograms on an annual basis,” advises Dr. Audrey Gonzalez, MD, a family medicine physician with STHS Clinics. According to the National Breast Cancer Foundation, Inc., approximately 30% of all new cancer diagnoses in women will be breast cancer, and there are over 4 million breast cancer survivors in the U.S. Even with tremendous advancements in care, many breast cancer patients face challenging side effects from their treatment, including chemotherapy-induced peripheral neuropathy (CIPN) and chemotherapy-induced cognitive impairment (CICI), which can negatively affect their care and impact their quality of life. As of now, the exact cause of breast cancer is still unknown. What is known are the risk factors that can increase the likelihood of developing breast cancer, namely: 1. Smoking and exposure to secondhand smoke 2. Poor diet (high in fat and low in fiber, containing preservatives/coloring agents) 3. First menstruation at age under 12 4. Menopause (cessation of menstruation) after age 50 5. Having the first child after age 35 6. Never breastfeeding 7. Having undergone surgery on the breast due to benign or malignant tumors 8.
