Integrated and Responsive Care: Ensuring healthcare is tailored to patient needs, reducing re-work, drudge, and administrative burdens on healthcare professionals. 2. Mitigating Risks: Addressing gaps, delays, and poor communication during transitions to improve patient outcomes. 3. Data Silos: Breaking down data silos and fragmentation across jurisdictions and care settings. 4. Connecting Care: Bridging connections outside of the hospital sector, creating the "glue" that binds hospital data with GP and allied health data. 5.
