This is an important source of information about patient experiences. So much to note, in particular the patient safety data. But I was really struck by 15.3% saying they didn't have enough information about how to manage their condition on discharge - something very simple which could impact readmissions, successful follow up on risk factors, medication safety & so much more!  The My health, My voice resources may make a difference here. They encourage us to ask 3 questions as patients and encourage providers to use them also  - What do I need to know now? What do I need to do next and why? What can I expect?
