ACLR Rehab: Don't forget about psychological readiness 🧠 So often we make our ACL-ers jump through hoops (pun completely intended) to get back to their pre-injury sport. Strength tests, hop tests, vertical jump tests are commonly what we'll focus on in return to sport planning. But don't forget about assessing psychological readiness to return to sport with the ACL-RSI questionnaire. ACL-RSI is a valid and reliable outcome measure in ACL populations and one recent paper by McPherson et al 2019 showed that scores >77% had lower risk of sustaining a 2nd ACL injury within 2yrs compared to those who chose to return to sport with scores of 76% or lower. Achieving "optimal" body angles to accelerate on the field while changing directions or performing agility like interventions when playing sports is crucial for sporting success. There are a lot of different interventions that can teach patient athletes acceleration mechanics in the sagittal, frontal, and transverse planes, however. The easiest interventions usually involve environmental constraints that limit the degrees of freedom a patient athlete can engage in from a movement perspective. Gradual progression to more "open" or chaotic environments with unlimited degrees of freedom then become the next natural movement progressions for patient athletes, but only once they master the basics! Since most all patient athletes have access to open wall space, the intervention below is a good starting point for them to practice their change of direction work in a closed and controlled environment. The wall is used here as an external cue for the patient athlete to dissociate their hips from their trunk, which when looked from a different perspective, can be considered a form of "hip" mobility too.
