I think there are multiple factors here, Overloaded no, Incorrectly loaded yes. Lack of maintenance axle parted from truck. Driver error for not refusing the task. The person loading the truck is also at fault.

NAH he's all good the load stayed attached to the deck. not his fault the deck came off the chassis! 😶
