This process helps to ensure that the solutions we provide are aligned with the actual needs of the community, rather than assumptions. By engaging with stakeholders and gathering data, we can prioritize resources effectively and create a sustainable impact. Recently, I had the opportunity to launch two health needs assessments. These assessments are invaluable in identifying gaps in healthcare services and understanding the unique challenges faced by different populations. They also serve as a foundation for developing targeted interventions that truly make a difference. In the world of social needs screening, sometimes (often, really) a meaningful conversation is all that we can offer. For many areas on your screening questionnaires, patients and families may simply find that the resources, services and supports they need are not available. This mismatch between needs and services may stem from ineligibility, already-full programs, cultural and language barriers, or the simple absence of appropriate resources. It would be better if we could always follow up on positive screening results with material assistance. Yet many times, a supportive conversation is all we can offer. Despite this challenge, there is evidence that supportive relationships with social service navigators can improve health, even when patients are not ultimately connected with resources. Many health professionals express hesitation to screen for issues they cannot address.
