How Interoperability optimized health outcomes 👉 Improved care coordination Interoperability allows healthcare providers to access comprehensive patient data in real time across different healthcare systems. 👉 Personalized and Proactive Care Interoperability allows providers to access detailed health records, which makes it easier to develop personalized treatment plans. It also allows for monitoring health patterns across patient populations allowing to identify at-risk patients earlier and offer preventive care. 👉 Patient Empowerment and Engagement Interoperability empowers patients by allowing them to access and share their medical information with multiple healthcare providers through patient portals or health apps. Patients can actively participate in their care by sharing relevant information with providers and receiving more personalized recommendations. 👉 Reduce Medical errors Interoperability helps reduce human error by ensuring that accurate and up-to-date information is available for every provider. With access to a patient’s complete medication history through interoperable systems, healthcare providers can prevent medication errors, such as over-prescription or harmful drug interactions. #interoperability #healthcare

Integrating digital health initiatives within your pharmacy is one way to show patients that you're willing to meet their needs. "In today’s healthcare landscape, patients are looking for accessible, affordable solutions, especially those who may be uninsured, underinsured, or unable to reach a healthcare provider due to logistical challenges." At IPC, we believe our digital health initiative, iCare+, may be exactly what your independent pharmacy needs. iCare+ offers pharmacies a way to serve patients who might otherwise forgo essential health services due to a lack of coverage. This tool offers affordable telemedicine and virtual care options, meaning that uninsured patients can now receive the care they need. It’s an important and proud day Surescripts as we release the 2023 National Progress Report that shows, along with the Network Alliance, how we’re advancing interoperability at scale: last year we saw a nearly 10% increase in patient intelligence sharing or nearly 24 billion exchanges of patient clinical and benefit information and our network connected 2.14 million care providers and reached more than 99% of the U.S. population. But it goes beyond the numbers – I’m proud of the impact that interoperability at this scale means for patients and those who care for them. Like when our technology saved patients an average of $37 per prescription and $785 per specialty medication and helped them get their needed medications faster. And what it means to have a clear view of clinician burnout and the impact it is having on patient access to care: 62.9% of U.S. counties had fewer than one primary care provider (PCP) for every 1,500 residents, and 196 had no primary care providers at all in 2023. The numbers also reveal where opportunity exists – highlighting the top 10 counties most impacted by worsening PCP shortages but where pharmacies could fill in the gaps to ensure patients have timely access to quality healthcare. This data also helps us identify where our innovations can have the greatest impact, empowering clinicians to collaborate more meaningfully as responsibilities shift across the care team. The report shows a variety of care providers are taking on more clinical responsibility—including pharmacists, among whom e-prescribing jumped dramatically in 2023. There’s a lot to unpack in this report and I look forward to sharing more at HIMSS Global Health Conference & Exhibition #himss24 this week and in the days and months that follow. I hope you’ll check it out.

The landscape of health care is rapidly evolving, and two key areas leading this transformation are Utilization Management and Case Management. Notably, physicians are finding value in the flexibility of remote work offered by Utilization and Case Management, contributing to a reduction in burnout rates among medical practitioners. However, the impact of these practices extends far beyond individual physicians. They play a crucial role in improving the quality of care and resource management within the healthcare insurance industry. By leveraging data-driven insights and streamlined processes, these areas empower organizations to optimize resource allocation, enhance patient outcomes, and drive operational efficiencies. #utilizationmanagement #casemanagement #medicalregulation

Forced consumerism in healthcare, driven by patients having increased “skin in the game,” has not generated tangible systemic value but has led to fragmentation. Patients face increasingly expensive choices for potential treatment paths without clear guidance on the quality or value of those options. This information asymmetry forces patients to navigate a complex web of providers and services to discover the most affordable option within the constraints of provider networks and referral patterns. They also face the added challenge of evaluating direct-to-consumer options, which may be more affordable but often exist outside of the “traditional” healthcare system. The complexity of this task is further compounded by the lack of price transparency in the U.S. healthcare system. This week, we examine how fragmentation within the healthcare system creates gaps in care and inconsistent quality, fostered by forced consumerism (Trend 6).

𝗪𝗵𝘆 𝗠𝗲𝗮𝘀𝘂𝗿𝗶𝗻𝗴 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗢𝘂𝘁𝗰𝗼𝗺𝗲𝘀 𝗠𝗮𝘁𝘁𝗲𝗿𝘀 ? The healthcare industry strives for a quadruple aim: 𝗵𝗮𝗽𝗽𝗶𝗲𝗿 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀, 𝗵𝗲𝗮𝗹𝘁𝗵𝗶𝗲𝗿 𝗰𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝗶𝗲𝘀, 𝗹𝗼𝘄𝗲𝗿 𝗰𝗼𝘀𝘁𝘀, 𝗮𝗻𝗱 𝗹𝗲𝘀𝘀 𝗯𝘂𝗿𝗻𝗼𝘂𝘁. But achieving this requires continuous improvement, and that's where measurement comes in. 𝗜𝗺𝗮𝗴𝗶𝗻𝗲 𝘁𝗿𝘆𝗶𝗻𝗴 𝘁𝗼 𝗶𝗺𝗽𝗿𝗼𝘃𝗲 𝘀𝗼𝗺𝗲𝘁𝗵𝗶𝗻𝗴 𝘄𝗶𝘁𝗵𝗼𝘂𝘁 𝗸𝗻𝗼𝘄𝗶𝗻𝗴 𝗶𝗳 𝗶𝘁'𝘀 𝗮𝗰𝘁𝘂𝗮𝗹𝗹𝘆 𝘄𝗼𝗿𝗸𝗶𝗻𝗴.Measurement is crucial in healthcare - it's like a progress report, telling us if changes we make lead to better patient care. Healthcare accreditation bodies set national standards for these measurements, involving input from various stakeholders like doctors, patients, and government agencies. These standards ensure the measurements: ✔ 𝗙𝗼𝗰𝘂𝘀 𝗼𝗻 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝘀𝗮𝗳𝗲𝘁𝘆 𝗮𝗻𝗱 𝗾𝘂𝗮𝗹𝗶𝘁𝘆 𝗼𝗳 𝗰𝗮𝗿𝗲. ✔ 𝗟𝗲𝗮𝗱 𝘁𝗼 𝗽𝗼𝘀𝗶𝘁𝗶𝘃𝗲 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝗼𝘂𝘁𝗰𝗼𝗺𝗲𝘀. ✔ 𝗔𝗱𝗵𝗲𝗿𝗲 𝘁𝗼 𝗿𝗲𝗴𝘂𝗹𝗮𝘁𝗶𝗼𝗻𝘀. ✔ 𝗔𝗿𝗲 𝗮𝗰𝗰𝘂𝗿𝗮𝘁𝗲 𝗮𝗻𝗱 𝗲𝗮𝘀𝘆 𝘁𝗼 𝘁𝗿𝗮𝗰𝗸. Clear measurement helps healthcare continuously improve and deliver the best possible care. Think of it like checking progress on a project. By tracking what happens after treatment, we can: 𝗟𝗲𝗮𝗿𝗻 & 𝗜𝗺𝗽𝗿𝗼𝘃𝗲: Hospitals can identify areas for improvement and learn from best practices around the world. 𝗗𝗲𝗹𝗶𝘃𝗲𝗿 𝗕𝗲𝘁𝘁𝗲𝗿 𝗖𝗮𝗿𝗲: Doctors use this data to make the best decisions for each patient and streamline care delivery. 𝗜𝗻𝗰𝗿𝗲𝗮𝘀𝗲 𝗧𝗿𝗮𝗻𝘀𝗽𝗮𝗿𝗲𝗻𝗰𝘆: Standardized measurements allow patients and others to compare hospitals and see their performance. 𝗩𝗮𝗹𝘂𝗲-𝗕𝗮𝘀𝗲𝗱 𝗖𝗮𝗿𝗲: Outcomes data links payments to positive results, incentivizing high-quality care. 𝗦𝗵𝗮𝗿𝗲𝗱 𝗗𝗲𝗰𝗶𝘀𝗶𝗼𝗻-𝗠𝗮𝗸𝗶𝗻𝗴: Patients can make informed decisions with a clear understanding of potential treatment impacts. #healthcare #patientcare #outcomes #qualityimprovement #patientcentriccare #healthcareadministration #clinicalexcellence
