🌟 Expanding Long-Term Services: A Vital Step for Proactive Care 🌟 As the Vice President of Information Technology for a nonprofit continuing care retirement community, #GivensCommunities ; I’m passionate about leveraging technology to drive meaningful change in how we care for older adults. This recent article from McKnight's reinforces a critical truth: ✴️ Expanding long-term services is no longer optional. it’s essential. ✴️ This study highlights the growing need for innovative approaches to meet the complex needs of older adults, especially in areas like enhanced home care, integrated health services, and proactive chronic disease management. Innovative approaches that empower individuals to age comfortably and independently in their homes and communities, free from pain and stress, for as long as possible. Technology plays a pivotal role in making these expansions feasible and effective, from telehealth solutions to predictive analytics that identify risks before they escalate. This article also points out the unfortunate challenges that face funding this technology. Medicare doesn't cover these services at all and programs like the CARE act offer very little funding to improve communication between health providers and family caregivers. At our communities, Residents, Team Members, and Leadership are committed to being part of this evolution, Finding funding to adopt these tools, and creating strategies that empower both residents and staff. By embracing proactive, tech-driven solutions, we can ensure that long-term services not only expand but also excel in delivering compassionate, personalized care. Collaboration is essential in the LPC industry to redefine what’s possible in senior living and healthcare. What steps are you taking to expand and enhance services in your organizations? I’d love to hear your thoughts! #LongTermCare #ProactiveHealth #HealthTechInnovation #leadingAGE #LeadingAgeCast https://lnkd.in/eRYM4BuC

🔑 Sharing 2 of the top priorities for health systems from this great thought piece by my colleague, Kim Radel! 1️⃣ Establish a senior-focused care model that leverages frameworks like the 4Ms (What Matters, Medication, Mentation, Mobility) and innovative approaches such as the GRACE model to streamline care across the continuum. 2️⃣ Prioritize systemwide medication review processes and use predictive analytics to proactively address risks, reduce unnecessary hospitalizations, and comply with CMS’s age-friendly measures. While easier said than done, a thoughtful, geriatric-focused strategy is critical to delivering high-quality, seamless care for seniors—while helping health systems achieve their clinical, financial, and market goals. Please commit to creating an age-friendly healthcare environment that puts older adults and their caregivers first. What strategies have you seen succeed in managing care for seniors? I'd love to discuss! #SeniorCare #HealthcareInnovation #ValueBasedCare

A comprehensive study conducted by Rutgers Health, which analyzed millions of Medicare beneficiaries' records, has identified nine distinct end-of-life care trajectories that older Americans typically follow in their final three years of life. This significant research underscores the critical importance of understanding these patterns, as they provide valuable insights for tailoring interventions that are aligned with individual patient preferences and needs. As the demand for home-based services continues to escalate, it becomes increasingly essential for healthcare professionals and organizations to recognize and adapt to these care trajectories. Policymakers need to address the projected 47% increase in the U.S. elderly population by 2050. Only 10.9% of Medicare decedents utilized advance care planning services from 2017 to 2018. Enhancing access to these services can enhance caregiver satisfaction and reduce healthcare costs. Check out this insightful article by Holly Vossel. #advancecareplanning

I was honored to participate in this year's White House Minority Health Forum hosted by the White House Office of Science and Technology Policy. In the session on "Making Innovation and Research Work for Everyone," I shared three key insights about family caregiving with attendees and health leaders from across the Biden-Harris Administration: 💡Family caregivers play a critical role in supporting patients navigating complex care (e.g., cancer, transplant, Alzheimer's), yet their needs are often overlooked. This lack of support can exacerbate existing health disparities. 💡Medicare and Medicaid can play a crucial role in strengthening the healthcare system by involving and assisting family caregivers. To fully leverage this potential, the Centers for Medicare & Medicaid Services should prioritize educating beneficiaries and providers about new Caregiver Training Services reimbursement codes and expanding in-home services and support for caregivers. 💡Paid family and medical leave isn't just a social policy, it's a health equity and economic inclusion policy. I'm grateful for the Administration's unwavering commitment to health equity and for including the National Alliance for Caregiving in this essential conversation. By supporting family caregivers, we can improve health outcomes and move the needle on eliminating healthcare disparities. Special shout out to Anabella Aspiras and Catharine Young, Ph.D. for being such gracious hosts! #Medicare #Innovation #Science #Research #ResearchDiversity #diversityequityinclusion #clinicaltrials #Medicaid #Healthcare #Health #Medicaid

ianacare co-founder Jessica Nam Kim shared in a recent Fortune article what needs to change for programs like #hospitalathome to reach their full potential and become the catalyst for effective home-based care models. Care is a team effort, and when home becomes the primary setting for critical and acute care, as in Hospital at Home, family members and other caregivers must be involved on day one. Jessica explains why below, and you can read the full article here: https://lnkd.in/e6jk2nji
Thanks to Fortune for publishing my Op-Ed to raise awareness of the critical missing piece of the hospital-at-home program… incorporating caregivers to ensure the last mile of patient care. What I wrote reflects the staggering reality of our care system AND my recent firsthand experience going through the hospital-at-home program as the caregiver for my dad with congestive heart failure. To be super clear, we NEED these programs like hospital-at-home to work. I'm a believer and advocate. However, it'll be impossible to reach its full potential without incorporating the family caregiver. Below, I share why and what we can do about it. The time is NOW to make this change in the system. The Hospital-at-Home program is the gateway to more care in the home programs. Let's not repeat what happens today in health systems where the last mile of care for the patient is unsupported. ianacare https://lnkd.in/dPqM4qQ7 #caregiversupport #digitalhealth #hospitalathome

It’s the eternal question: how can we deliver higher quality care at lower costs? It turns out person-centered care may be the answer. The buzz around person-centered care is usually focused on how it improves the care experience and brings dignity to the aging. But the side you don’t often hear is how maintaining a person-centered care climate actually makes good business sense. It can reduce costs, for the care recipient and the care provider, while improving outcomes. Yes, you read that right: it can cost less, but achieve more. A key reason is that this model of care can keep older adults engaged in their treatment plan, which in turn helps them age in place and delay or avoid moving to higher-cost settings. And it can improve staff retention, reducing hiring and training costs. So let’s take a look: https://lnkd.in/eWCT_5Gu #personcenteredcare #agingcare #longtermcare
