PULSATING EARDRUM - WHAT IS THE CAUSE? Why is your chosen visualisation method for #earwaxremoval critical? This is because removing #earwax can reveal potential underlying pathologies which can go easily undetected with poor visualisation. Upon removal of this dry occluding #earwax and #deadskin using the high #magnification #WAXscope, this patient had a visible posterior #pulsating #eardrum which was quicker and not in synchrony with their #respiration. Indeed, the pulsating continued even when the patient held their breath. As such, it’s not due to a #patulous #eustachiantube. Therefore, this patient has been referred onwards to #ent since they may have a tegmen tympani dehiscence (TTD) or #paraganglioma (jugular glomus). The video was too short to fully explain everything but they had a normal Type A tympanogram in this their left ear. Weber was lateralised to their worse right ear where they have a severe longstanding attic retraction causing an auto-atticotomy and conductive hearing loss. Rinne was negative bilaterally but may have been false negative in the left due to their right conductive hearing loss and BC being heard louder in this ear. No other symptoms including no audible pulsatile #tinnitus in their left ear.
