🎉 Exciting News! 🎉 We are thrilled to share a remarkable case report published in the July 2024 issue of Cath Lab Digest, titled "Successful Dilation of a Heavily Calcified LAD With 3 Layers of Under-Expanded Stents With the OPN NC Super High-Pressure Balloon." In this report, the OPN NC Super High-Pressure PTCA Balloon was used to tackle a challenging case involving a heavily calcified left anterior descending (LAD) artery with three layers of under-expanded stents. Highlighting constant improvements and innovative strategies, Boston Children's Hospital cardiac team has significantly advanced the safety and effectiveness of the Ross procedure for aortic valve replacement. Emphasizing tailored approaches to manage and prevent complications, their technique now offers a durable solution for patients of all ages. Recent data highlights this success: over the past 17 years, only one of 58 patients who weighed more than 40 kilograms required further surgical intervention for the new aortic valve post-Ross procedure, and complications were minimal even among those who had previous heart surgeries or needed additional procedures during the Ross operation. In the last few posts I have shared the high-yield and "low-hanging fruit" lung #ultrasound applications for a novice #pocus user. Today is the final lung #ultrasound application: identifying pleural effusions (fluid in thoracic cavity). Ultrasound is nearly 100% sensitive in detecting even small amounts of fluid in the thoracic cavity. Chest x-ray is notoriously lousy, especially when performed on supine patients. The 2024 meta-analysis linked below found that lung ultrasound sensitivity for pleural effusion was 95% compared to 68% with chest x-ray. What are the key findings on #POCUS for pleural effusion? 1. Loss of mirror image artifact (liver or spleen reflecting over the diaphragm). 2. Presence of spine sign (spine is visualized superior to the diaphragm). 3. Fluid is seen. Transudative fluid appears anechoic (black). Exudative fluid has echos and debris in it. Other cool things about using lung #ultrasound for pleural effusions: 1. You can estimate the size of the effusion. 2. You can use it for guidance during drainage to decrease complications (bleeding and pneumothorax). More on this in a future post! I'll do some cost calculations for you all. It's quite impressive what iatrogenic pneumothorax costs. 3. You can check for a pneumothorax immediately post-procedure instead of waiting for a chest X-ray to be performed. Remember, chest x-ray is also an inferior test for pneumothorax detection.
