In this episode, interventional cardiologist Dr. Peter Soukas joins us to discuss percutaneous transmural arterial bypass (PTAB) using the DETOUR system, a novel percutaneous treatment for extensive and complex blockages in the superficial femoral artery (SFA). Dr. Soukas shares his extensive experience and insights into PTAB development, benefits, patient selection, and procedural details. The idea behind PTAB is to create an SFA-to-popliteal artery bypass through endovascular means, using a system of overlapping stent grafts within the femoral vein as a conduit. 𝗙𝗶𝗿𝘀𝘁 𝗖𝗮𝘀𝗲: 𝗜𝗻𝘁𝗲𝗿𝗿𝘂𝗽𝘁𝗲𝗱 𝗖𝗼𝗮𝗿𝗰𝘁𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝘁𝗵𝗲 𝗔𝗼𝗿𝘁𝗮 (𝗖𝗢𝗔) Excited to share a complex case we recently handled, showcasing our team's expertise and innovative approach to interventional cardiology. 𝗖𝗮𝘀𝗲 𝗢𝘃𝗲𝗿𝘃𝗶𝗲𝘄: A simultaneous contrast injection via the right femoral and left radial artery revealed an interrupted coarctation of the aorta with well-developed collaterals. 𝗣𝗿𝗼𝗰𝗲𝗱𝘂𝗿𝗲 𝗗𝗲𝘁𝗮𝗶𝗹𝘀: • Retrograde wiring initially failed. • Success was achieved via the left radial access using a coronary wire, followed by balloon angioplasty with a coronary balloon. • The wire was snared, and a pigtail catheter was advanced retrogradely beyond the stricture. • A stiff wire was then crossed, and a self-expanding stent was deployed. Post-dilation was strategically postponed for several months to ensure optimal patient outcomes. • This case highlights the importance of adaptability and precision in interventional cardiology.
