PBMs are stealing from health plans Here's one of the most nefarious games It gets bonus points for destroying independent pharmacies The largest PBMs are also owned by the largest insurance companies They also own the pharmacy distribution chain And someone else pays all their bills (health plans) So here's the game The PBM "negotiates" a very high reimbursement rate for the pharmacies it owns It simultaneously hammers independent pharmacies with paltry rates and all the claw backs and gag clauses Then it charges the health plan for the ridiculous reimbursement it pays itself And, as if by magic, all the extra expenses get eliminated through intercompany eliminations Fun right? It’s not stealing. The affiliated PBM charges are designed to raise the plans MLR so the excess money goes to an affiliated company’s profits vs MLR rebates. But what do I know

And, of course, many PBMs that are hammering independent pharmacies also happen to have parent companies that own chain pharmacies who compete with the independents. But I'm sure that's just coincidence.

Thank you for your work Preston Alexander Keep beating that pithy drum of yours. That sound you hear is independent community focused pharmacies closing by the droves. Employers (aka plan sponsors) need to wake up and start questioning what they are paying PBMs for.
