Ensuring accurate cervical length measurements during transvaginal ultrasounds. Extend your scan duration! Here's why: During a transvaginal ultrasound, a seemingly short cervix initially appeared on the screen. However, upon extending the scan beyond 3 minutes, it became clear this was a temporary shortening due to a uterine contraction. Key Takeaways for Accurate Cervical Length Assessment: 1. Incontinentia pigmenti in UWF full range SS-OCTA! A 32-year-old female presented with distorted and blurred vision in her right eye. 160° UWF full range SS-OCTA (montage of two 24mm x 20mm) revealed vascular dilation, tortuosity, and temporal vascular anastomoses (fig. A arrowheads) with extensive nonperfusion (fig. A asterisks) in both eyes. Incontinentia pigmenti was diagnosed based on abnormalities of retinal features, skin and teeth and family history. Foveal retinoschisis due to vitreomacular traction by the posterior vitreous cortex was detected (fig. B arrows) by high-resolution 24mm B-scan in her right eye. A 400kHz speed full range swept source OCTA (#BMizar, TowardPi Medical) was used in this case to detect peripheral anomalies. A ultrawide field of view above 200° can be achieved with flexible montage of 24mm x 20mm full range OCTA on the device. Are you in a clinic or preparing for exams …. When you test #ocular #movements remember to do this and consider the anatomy and potential #diagnosis behind the movement you assess. In the primary position perform an alternate cover test to assess the underlying phoria. So from the primary position ask the patient to follow your pen light into left gaze. Go as far as you can without losing a reflection, repeat an alternative cover test to see if the phoria has changed. Thus assess the function of the left lateral rectus (eso would increase) and the right medial rectus (exo would increase) Think is the sixth and third cranial verve working properly? Can you see an increase in height? If the right eye is higher think Inferior oblique overaction and possible right IVn palsy If the right eye is lower consider the present of right Browns syndrome If there was height in the primary position and if this is still present in left gaze think if about Skew deviation being present. If there is a mechanical limitation of movement consider orbital diseases such as thyroid eye disease, muscle problems like myositis, or blow out fracture. Remember to look at any palpebral fissure changes and think is Duane’s retraction syndrome is present or abberent regeneration (this is an ocular motor synkinesis post IIIn) If there is a failure of gaze or a slow saccade to the left consider the function horizontal gaze centre of the para pontine reticular formation. Testing vestibular movement will be useful. Look for nystagmus all types. Especially abduction nystagmus which may indicate a medial longitudinal fasciculus problem such as MS or post stroke. But this can often be the first sign of MG (myasthenia Gravis) remember to look for fatigue and variability. So you can glean a lot from left gaze now your ready to ask the patient to look to the right !
