Greater depth can be found in books such as the Six Sigma Handbook by Tom Pyzdek, Understanding Statistical Process Control by Donald J. Wheeler Ph.D., and David Chambers.

What is Root Cause Analysis? Root Cause Analysis (RCA) is a systematic process used to identify the underlying causes of a problem or failure. The goal of RCA is to determine how an event occurred, why it happened, and how to prevent it from happening again. It involves a detailed examination of the problem to uncover the root cause rather than just addressing the symptoms. Tools and Techniques Commonly Used in RCA:- - 5 Whys: Asking "why" repeatedly (typically five times) to drill down into the cause of a problem. - Fishbone Diagram (Ishikawa Diagram): A visual tool that categorizes potential causes of problems to identify root causes. - Pareto Analysis: A statistical technique that identifies the most significant factors contributing to a problem. - Failure Mode and Effects Analysis (FMEA): A method for evaluating processes to identify where and how they might fail and assessing the relative impact of different failures. - Fault Tree Analysis (FTA): A top-down, deductive failure analysis used to determine the causes of system failures. Key Components and Steps of RCA:- - Define the Problem - Collect Data - Identify Possible Causes - Analyze the Causes - Identify the Root Cause - Develop and Implement Solutions - Monitor and Evaluate - Document and Communicate For Example, 1. Production Delays --> Machine Breakdown--> Poor Maintenance In the above case, “Machine breakdown” is a symptom & we must deep down to find the Underlying causes like Poor maintenance & so on. 2. Poor Student performance --> Students are not studying well --> Outdated curriculum that does not engage students.
