Dr Minesh Patel, GP and NAPC Non-Executive Director attended the Frimley Health and Care Integrated Care System ‘Inspiring Primary Care Transformation’ event last week to lead two sessions on ‘Population Health and Communities – Integrated Neighbourhood Teams’. Minesh said: “We had a very engaging discussion on population health improvement and making neighbourhood care real, looking at the keys to success including working upstream and the importance of activating teams and activating residents. "We discussed examples of simple upstream interventions around common underlying drivers of health demand such as diabesity and effective and enabling wellbeing interventions such as singing groups, supporting people living with frailty and the impact and ended on the benefits of the group consultations model as a way of engaging and activating people with some examples from my practice. “Frimley is a dynamic ICS with some local challenges, but they realise the importance of improving working lives and are taking positive steps to doing this. They have well established population data and analysis that they are actively using which is really positive. I used the bank holiday weekend in the UK to listen to the International Foundation for Integrated Care (IFIC) Plenary 3 at #ICIC24, co-chaired by Professor Robin Miller and Sarah McLaughlin. The discussion centred around global experiences of general practice in delivering #integratedcare. As Professor Robin Miller challenged in his opening - in general practice we can see both the best and the worst of care integration. Starting with the best, Sarah McLaughlin from Mid & East Antrim Agewell Partnership talked to Billy Johnson about his lived experience of #personcentredcare that was based around what mattered to him and the confidence that gave him in his #generalpractice. It was such a lovely conversation to listen to between two people who clearly understood the value of #relationalcare. Amarjit Maxwell shared the experience of the one of the Primary Health Organisation THINK Hauora in New Zealand as they support the general practice journey to well-being partner for communities, Richard Lewanczuk shared the experience in Alberta, Canada as general practice moves to an extended team that includes people and care-givers as the main providers of care, and Jan De Maeseneer took us on a journey through 50 years of primary care development in Flanders towards Community Health Centres. So many lessons shared along the way. My takeaways: 📈 The pressure on general practice everywhere is increasing and unlikely to decrease anytime soon 🏘 General practice that is moving away from the 'traditional model' of reactive care to preventative and relational care will thrive and help their communities thrive BUT - the 🐘 in the room was 💶! - Investment in new models of multi-disciplinary primary care can accelerate this process hugely, as the evaluation of a model of care shared by Jan shows [https://lnkd.in/eHNmBsux] but this has not yet happened to any significant degree. Enhanced Care Management: • is a statewide MediCal Managed Care Plan benefit to support elevated comprehensive care management for members who have the most complex cases • assigns members with complex needs a Care Manager to help them navigate their care and services across the physical, behavioral, and dental health delivery systems. We’re working with California Health Policy Strategies (CalHPS) to support a CalAIM Statewide Dementia Care Learning Collaborative to support community-based organizations that are serving older dual eligibles and persons with dementia at high risk of institutionalization to offer enhanced care management.
