The TEFCA Common Agreement Version 2.0 has been published. Version 2.0 of the Common Agreement includes enhancements and updates to require support for FHIR API-based transactions. This support for standardized APIs will help to increase patient access to health information and facilitate easier exchange via TEFCA. Common Agreement Version 2 was just released this morning and is available on ONC’s website, which is a huge step forward for TEFCA (insert FHIR pun here 🤣). The same version of Version 2 will be published in the Federal Register soon. Some highlights of Version 2 include: - Facilitated FHIR exchange available b/t P/S to enable exchange b/t point-to-point APIs in addition to QHIN-to-QHIN IHE exchange; - Static Participant/Subparticipant Terms of Participation that will allow P/S to more easily incorporate CA flow-down provisions into existing data use agreements; - P/S may conduct TEFCA exchange in multiple QHINs; - Introduction of Principal/Delegate roles and requirements to allow certain organizations to conduct exchange on behalf of the Principal, which is responsive to existing market use cases and dynamics; - Streamlined Common Agreement. And much more! We will also be releasing additional resources in the coming weeks to help explain all of the changes in more detail. Stay tuned!

That's what a lot of people don't understand with FHIR. The international resources are not the gold standard to adopt as is, they are not MEANT to be the perfection all must follow without wavering a hair from, they are not even meant to be fully complete, even the few that are "normative" (meaning "completed"). They are a basic toolbox meant to be customized, adapted, enhanced to fit your country/use-case's needs. That's why one must first create national bottom up driven data models, then see how one can exchange those in FHIR and not the other way around, not take international FHIR resources as a national data model structure.
#FHIR follows the 80/20 rule: 80% of common use cases are addressed by the standard resources, but for the other 20%, we rely on customization. FHIR Profiles play a crucial role in this customization, ensuring that specific healthcare requirements are met without compromising #interoperability.
