It was a profound privilege to host this year’s Harvest of Hope celebration, where we honored the resilience and strength of Sacramento’s Slavic refugee community. Together, we distributed over 1,000 turkeys to families in need, providing nourishment and meaningful support during the holiday season. This year’s event carried special significance as it marked the first Thanksgiving for many families who fled war-torn regions in search of safety and a fresh start. Harvest of Hope celebrated not only their courage but also Sacramento’s shared commitment to welcoming and supporting these families as they rebuild their lives. In 2024, ModifyHealth and our community partners have delivered critical resources to thousands of families, including housing, nutritious meals, and access to essential services. A highlight of the event was the CalAIM Community Champion Awards™, where we recognized health plans for their extraordinary dedication to improving community well-being. These partnerships exemplify the impact we can achieve when we work together to address systemic health challenges. Through the California Advancing and Innovating Medi-Cal (CalAIM) program, ModifyHealth leads food-as-medicine initiatives to address social determinants of health. By delivering medically tailored meals to Medi-Cal beneficiaries, we are empowering underserved populations to better manage chronic conditions, reduce health disparities, and achieve meaningful health outcomes. What a way to finish the week! I’ve just come from “Strengthening Community Health - past history and implications for the future”. With nearly 100 Community Health leaders and 50 years of experience in the room, I learned more in 5 hours than I could have in 5 years on my own. Key Takeaways: 1. Community Health is extraordinary, delivering hundreds of key services for just 0.5% of the Victorian health budget. 2. Victoria’s system is a national gem. Other states are suffering from defunding Community Health. 3. Community Health is a key first responder in any crisis (anyone remember COVID, flood, bushfires?) 4. Imagine what would be possible if we doubled funding to just 1% of the Victorian health budget? The dissemination Workshop was long overdue as the Ministry of Health launched these documents in April 2023. Earlier this week, I was happy to speak at the World Health Organization Community Protection Partners meeting, which brought together local, community-based, civil society and partner agencies to discuss progress, gaps, and priorities for action in delivering strengthened community protection outcomes Over the years, we've made significant strides in recognizing the importance of engaging with communities and establishing mechanisms for feedback and collaboration. There are countless 'bright spots' that showcase the invaluable role communities play in prevention, detection, and response to health emergencies. However, despite progress in promoting community agency, true co-design with communities remains elusive. Despite repeated calls for greater involvement in planning and decision-making, practical challenges persist in translating these aspirations into everyday workflows and practices. The conversation generated numerous insightful questions and comments, underscoring the complexity of community engagement and highlighting the need for systemic changes to ensure that all voices are not just heard but are valued and utilized in decision making. I look forward to continuing the discussion and exploring how we can overcome these challenges together.
