'The myth of objectivity, which depends on a radical separation of the knower from the known, has been declared bankrupt. We now see that to know something is to have a living relationship with it-influencing and being influenced by the object known'. -Parker Palmer, To Know as We are Known as cited in Loving to Know "The more expansive and reverent paradigm of knowing as interpersonal covenant relationship allows for better knowing, knowing that is fuller-orbed and mutually person-enhancing and therapeutic."-Esther Meek, "Loving to Know" This is the path we have taken with Telos LTC in knowing our customers. To build a better product we must listen and grow along with those we hope help. There is then a ripple effect of assistance passed along to others in need. I believe this path will ultimately create lasting relationships, better products and better outcomes. Coming to know is a journey. A journey that is relational and it is binding, or as Meek calls it, covenantal. We hope to join you on your journey of knowledge and growth. #medicaid #longtermcare #nursinghome #healthcare

As healthcare evolves, the value of consistent and comprehensive primary care cannot be overstated. Evidence shows that when patients have regular access to primary care services, both the cost of care decreases and the quality of care improves. Vermont is leading by example in healthcare efficiency and quality through its embrace of the Patient-Centered Medical Home (PCMH) model. This approach provides consistent and comprehensive primary care, significantly reducing healthcare costs and improving patient outcomes. By prioritizing preventive care and chronic disease management, Vermont’s PCMH model ensures accessible and effective healthcare for all. Investing in robust primary care systems is an investment in healthier populations and more sustainable healthcare. Let’s advocate for accessible, continuous primary care for everyone as a fundamental step towards a more efficient and effective healthcare system. #Healthcare #PrimaryCare #PatientCare #HealthEconomics
According to a new report from Vermont's Blueprint for Health, medical and pharmacy claims are 30 percent lower for Vermonters receiving care through its primary-care practices than for those treated elsewhere--just one more proof point that primary care boost outcomes and lowers costs. Great to see this investment in Primary Care. In two years, the Labor government has increased Medicare rebates by twice as much as the former Liberal government did in its nine-year stint, through its $940 million in additional Medicare funding delivered last year and $900 million delivered this week, according to the Department of Health and Aged Care. The latest data published by the DoHAC showed 915,000 estimated additional free visits in May and an overall bulk-billing rate of 79% nationally. This marks a modest increase of 3.4 percentage points since the induction of the triple bulk-billing incentives. The biggest increases were seen in regional and rural areas, with an estimated 900,000 additional visits since the incentives, according to the government. While the RACGP championed the increases, GPs remained concerned that the low-ball MBS indexation of 3.5%, which took effect yesterday, would undercut any increase.
‘Modest’ bulk-billing rise, but band aids won’t fix ‘festering sore’ 📈 Concerns remain that the measly 3.5% indexation of Medicare will ‘erode’ any growth and that, 40 years on, Medicare isn’t fit for purpose. https://lnkd.in/gPN2qg5j #generalpractice #primarycare #familymedicine #medicare #bulkbilling

A stroke survivor, Marlene Nathanson, was on the road to recovery when her prior authorization request for treatment was refused under her Medicare Advantage plan. Unfortunately, Marlene’s circumstance is not unique. A recent The New York Times piece details Marlene’s encounter with a prior auth denial and its impact on her access to care. The article also features physician experiences with prior authorizations, noting that although new rules have been introduced, it comes down to enforcement at the local level to ensure real change is made. Prior authorization denials can have a significant impact on patient care, cause economic stress, and contribute to staff burnout. At Valer, our mission is to speed and simplify current prior auth processes to reduce denials and the need for appeals, helping to boost patient outcomes. After two decades, Vermont’s #BlueprintForHealth showing impressive results & the legislature is pretty happy with it. The #PrimaryCare centric framework (started at the behest of a Republican governor) is showing 30% lower per capita #MedicalCosts & 30% lower per capita #drug cost across the state. Funny thing: the more primary care you have, the higher the overall #populationhealth & the lower the per capita cost (AKA #healthcare-related wealth transfers) according to the report presented to the Vermont legislature. #TripleAim #QuadrupleAim #PublicPrivatePartnership #PublicPrivatePartnerships
