A quick search online reveals that, at least anecdotally, many patients behind their screens are suffering from symptoms resembling angina and anginal equivalents but feel that they are not being given serious attention as patients and thus suffer additionally from a lack of diagnosis (which then equates to a lack of treatment). Clinicians are only just now realizing how prevalent ischemia without any obstructed coronary arteries (INOCA) really is, but clinical practice continues to lag far behind with only a handful of institutions offering the ability to interrogate the microvasculature to get a "complete" picture of patient heart health. Worse still, almost all of our traditional non-invasive frontline tests are notoriously difficult to use consistently for diagnosing CMD at the beginning of a patient's care journey. Surfactant is a miracle drug. Used in NICUs every day. It has saved countless newborns. Surfactant = surface active agent. It’s like soap. It lowers surface tension. We all make it, but the cells that make it in preterm infants are immature. When you give it you often see a fairly rapid decrease in oxygen requirement. It is a life saver. Check out this story.
The story of pulmonary surfactant is one of the most complete bench ➡️ bedside ➡️ major public health impact stories in medicine (though as a pulmonologist I may be biased 🙂). Remarkably, John Clements discovered both the cause and synthesized the treatment, which has saved tens of thousands of lives. He passed earlier this month, at 101. Today marks a paradigm shift in the monitoring of #Type1diabetes (T1D). Our organisation, along with 60+ international experts & 10+ diabetes orgs, published new T1D monitoring guidance for health care professionals. Research has shown that the condition can exist without symptoms for months or years as early-stage T1D, before it is traditionally diagnosed. Until now, there was no standard advice on how to detect, follow up, or support people with early-stage T1D. For the first time, individuals, families, and healthcare professionals now have concrete guidance how to diagnose early-stage T1D, monitor progression of the condition, catch symptoms early to prevent diabetic ketoacidosis and identify those eligible for clinical trials. The guidance document was co-authored by JDRF-funded Australian researchers Jennifer Couper, Maria Craig, Kirstie Bell, John Wentworth and endorsed by the Australian Diabetes Society.
