73 yo male presented with progressive loss of grip function and weakened wrist/thumb/bicep and triceps strength. Pronation and supination was weak as well. Neck and arm/forearm pain not amenable to pain medication. He was offered posterior decompression, laminoplasty, multilevel hemilaminectomies, posterior fixation and multiforaminotomies by another colleagues. In my perspective, Loss of lordosis plus Lateral nerve stenosis due to uncus hypertrophy have always been an indication for anterior approach. Even though aggressive uncinatectomies is a powerful technique I regurarly use in arthroplasty cases, It can also be used in ACDF and AS cases to achieve wide nerve decompression and “disengagement” to aid lordosis restoration. Patient recovers well and discharged home on POD: 2 while his pain reduced remarkably. 4 level ACDF should be monitored closely for later posterior supplemental fixation. Just like your blood, there are fluids in the human eyes that are synonymous with volume and pressure. This is what brings about a phenomenon called Intra-ocular pressure(IOP). Intra-Ocular Pressure is the pressure exerted by fluids within the eyes. IOP can be caused by so many factors such as: -Increased production of aqueous humor in the eye. -A blocked drainage angle that prevents aqueous humor from exiting the eye. -Injuries to the eye. -Side effects from some medications like corticosteroids. It is important to note that the optimal range for eye pressure or IOP is 10-20mmHg. Anything higher than this can lead to glaucoma. IOP is determined by tonometry, a test conducted by an optometrist to ascertain the pressure within your eyes. Unlike a high blood pressure, a high eye pressure has no actual symptom. You may never know if you have eye hypertension until it deteriorates into a clinical condition like glaucoma which presents its own symptoms. Hi I am to share about an article Damping. Damping is a phenomenon that occurs during cardiac catheterization when the catheter's outer diameter is larger than the ostial diameter or when the catheter tip touches the vessel wall: Characteristics Damping is characterized by a sudden drop in mean coronary pressure, a narrow pulse pressure, and a delayed upstroke and downstroke. Causes Damping can be caused by: The catheter's outer diameter being almost as large as or larger than the ostial diameter The catheter tip being pressed against the vessel wall Significance Damping can indicate severe partial obstruction and adverse catheter placement. It can lead to complications such as ischemia, chest pain, and arrhythmia if not identified quickly and the catheter repositioned. Reversible causes The most common reversible cause of damping is catheter-induced coronary spasm. #snsinstitutions #designthinking
