REVERSE Thumbs-Up 👍 Restoring full active internal rotation after reverse shoulder replacement remains challenging, even for many experienced shoulder surgeons. Correct placement of the baseplate and glenosphere following balanced reconstruction and proper glenoid lateralization—without causing acromial overtension or neural overstretching—is essential, along with ensuring balanced humeral retroversion. Subscapularis repair is still debated, but I always aim for a tension-free SSC repair to protect the conjoined tendon from rubbing and fraying against the humerus, allowing smooth rotation of the humerus behind the tendon. The last patient in this video had his left shoulder reversed three years ago, and now it’s time to operate on the right one. my ‘Fixed Reality’ in Shoulder Arthroplasty I’ve been implanting shoulder prostheses for several years, and by now, I know the process so well that I could perform certain steps with my eyes closed. However, there’s one part of the process I never skip before any operation: preoperative planning using 3D CT software. Without this detailed preparation, I wouldn’t be able to achieve the precision I strive for. Despite my experience, each shoulder joint presents a unique set of challenges—bone and tendon damage, pathological torsion, version, inclination, bone spurs, cysts, and more. This 3D planning isn’t just a rough guide; it’s the operative reality. I print it out, pin it to the wall in the operating room, and refer to it two or three times during surgery to ensure proper bone resection and placement of both the glenoid and humeral components based on the joint’s morphology. REVERSE Thumbs-Up 👍 Internal Rotation After RSA: The challenge of implanting a reverse shoulder prosthesis is undeniably demanding for shoulder surgeons. Patients present with torn rotator cuffs and worn, deformed joints, yet their goal remains to regain full range of motion and improve their quality of life. It’s a tough battle against nature’s anatomy. However, the deep satisfaction we feel when our patients regain full shoulder movement after surgery keeps us motivated to take on this challenge time and again. For instance, the last patient had his left shoulder reversed 3 years ago, and now it’s time for the right one. Restoring full active internal rotation after RSA remains challenging, even for many experienced shoulder surgeons. Achieving balanced reconstruction and proper glenoid lateralization—without causing acromial overtension or neural overstretching—is essential, along with ensuring balanced humeral retroversion. Buoyed by an impressive 2024 so far, Nerve Tape could become one of VCU’s most successful licensing deals ever through its innovation pipeline. The pioneering technology was developed by Jonathan Isaacs, M.D., a professor in the VCU School of Medicine’s Department of Orthopaedic Surgery and chair of VCU Health’s Division of Hand Surgery. He wanted to improve nerve repair, whose traditional suturing techniques required precise alignment and significant time. Isaacs envisioned a simpler solution: a biologic wrap to streamline and enhance the nerve repair process.
