What Is Patient Experience? Introduction Patient Experience Defined Patient experience encompasses the range of interactions that patients have with the healthcare system, including their care from health plans, and from doctors, nurses, and staff in hospitals, physician practices, and other healthcare facilities.Patient experience includes several aspects of healthcare delivery that patients value highly when they seek and receive care, such as getting timely appointments, easy access to information, and good communication with health care providers. Understanding patient experience is a key step in moving toward patient-centered care. By looking at various aspects of patient experience, one can assess the extent to which patients are receiving care that is respectful of and responsive to individual patient preferences, needs and values. Evaluating patient experience along with other components such as effectiveness and safety of care is essential to providing a complete picture of health care quality. Difference Between Patient Experience and Patient Satisfaction Satisfaction is about whether a patient’s expectations about a health encounter were met. Two people who receive the exact same care, but who have different expectations for how that care is supposed to be delivered, Patient experience on the other hand is finding out from patients whether something that should happen in a healthcare setting (such as clear communication with a provider) actually happened or how often it happened. Surveys Measuring Patient Experience While there are various ways to gather information on patient experience, some surveys do not ask patients how satisfied they were with their care; rather, they ask patients to report on the aspects of their experiences that are important to them and for which they are the best, and sometimes the only source of information. Because the surveys ask well-tested questions using a consistent methodology across a large sample of respondents, they generate standardized and validated measures of patient experience that providers, consumers, and others can rely on. Relating Patient Experience to Other Quality Measures A positive patient experience is an important goal in its own right. Moreover, substantial evidence points to a positive association between various aspects of patient experience, such as good communication between providers and patients, and several important healthcare processes and outcomes. These processes and outcomes include patient adherence to medical advice, better clinical outcomes, improved patient safety practices, and lower utilization of unnecessary healthcare services. How do direct primary care (DPC) physicians help their patients navigate a greedy healthcare system while ensuring they get the care they need? Here’s a real life example of what we do at Zuli Direct Care: A patient needed an imaging test and has a high-deductible with HSA plan. We informed the patient about the price for this test through the local healthcare systems: Health System A: $1400 Health System B: $7100 Health System C: $4900 Then we looked on the Radiology Assist website and see that same imaging test is $438. That’s not a typo or missing a 0. It’s $438. The downside? The nearest Radiology Assist location is 1 hour away instead of down the road. Imagine if this patient had a doctor at Health System B. They wouldn’t think twice about having their imaging test scheduled at Health System B because our culture has normalized that you get your test scheduled with the same organization as your doctor. “Our scans are better.” or “We will get your test results quicker.” Have you heard those persuasions? Don’t believe them. This is the ugly greed of healthcare in America on full display. Take home lessons: Do not assume you must get your labs or imaging tests at your local health system. If your physician is an employee of a health system, he or she may be unwilling to help you find a lower cost option. We as physicians, surgeons and clinicians need to take more ownership. We need to be thoughtful and creative in architecting opportunities where we really own our decisions and the benefits (or drawbacks) of them. Aligning physician and institutional incentives has never been easy, but access to cost and margin data via Avant-garde Health has given us an edge. Having successfully built and pioneered the practices outlined in this article off of AGH’s data, I thought of 5 more important points to share that can help get this off the ground for other institutions. 1st- Achieving success with a plan like this is all about how you deliver the opportunity- to both the clinical team and your institution’s financial stewards. It’s just that, an opportunity for both the clinicians and health system to win by working together towards a shared benefit. 2nd- Manage and build reasonable expectations and goals and show what things will look like if this approach isn’t realized as well. This helps a lot. 3rd- Have a plan for reassessment and adjustment if those goals are being exceeded or not reached. 4th- Every surgeon/physician in the dept shares in the incentive equally. This way, the MDs who were already doing better with costs still see a benefit and can become champions leading the others down the path of enlightenment. 5th- Go big with the incentive but just not too big! The incentive needs to be enough to matter daily, but not so much it can lead people astray from doing their best work possible. Ultimately the patient outcome needs to outweigh the incentive. This is difficult for anyone but a physician leader to truly understand. I am passionate about getting value based care right (and have yet to find the place where that’s been done yet). 📲 Technology alone can’t lead primary care physicians to success in value-based care. Case in point, starring Nancy Rivera: She’s been putting in serious miles along the bayous of Louisiana, tying up some loose ends with some of our new practices to enroll them in our care management program, scheduling help and clinical documentation assistance. 🩺 One particular practice wasn’t sure they wanted our assistance with scheduling Medicare Annual Wellness Visits. They were already essentially outsourcing AWVs through a contractor. Nancy showed them their data. Their completion rate was below 1%. For reference, Wellvana's AWV goal is 90%. That prompted a deeper discussion about the importance of being engaged with preventive care and making sure Medicare patient conditions are updated yearly with the goal to build personalized care plans. Understandably, these are difficult conversations about workflow and effectiveness. Because of the trust Nancy had built with the provider, they listened. And now their practice and patients are benefiting with more support in preventive care efforts. Without human-to-human relationships, we have a much tougher time inspiring the behavior change value-based care requires. We love to tell success stories about practices hitting their AWV goals or the success of our discharge management program or our nurses role in improving the lives of patients between appointments. But before the clinical wins, it takes a team to lay the groundwork for practices so they can succeed in this new world of value-based care.
