I always work with such philosophy, i always support what is the right eventhough if some plant head or CEO is front of me. One day plant head rejected some sprocket due to bore issue provided by vendor and same sprocket before was manufactured inhouse which was rejected after inspection but they have utilised it bypassing the quality. I directly wrote the mail to him that quality is not as per convenience it should same for everyone. But everyone supported that plant head, so i resigned from that company. I don't want to work in the company culture where wrong practices are carried out. If there is any mistake of installation team they should warned or fine the respective team not blame the vendor or contractor. Root cause analysis to be carried out properly. The success of the company depend upon the management mentality.

This is everything about knowledge. If you got the knowledge then you will do the right thing and not continue doing a wrong trend. If you don't have the knowledge then you will tend to do what other people doing (trend). Simple as that. Knowledge is everything and clears all misconceptions. My advice is, if you are uncertain, not confident about the right way to do something, then please always ask for advice. It is not a shame to ask! It is a shame to put your ego first and continue doing something in a wrong way, that endangers to create more issues further down the line. Be humble, and ask, learn and embrace from other people's experience, knowledge and point of view. You are not supposed to know everything in this world. Don't let your ego destroy your path to excellence.

Who decides?

True. But unfortunately it is not always clear what is right and what is wrong, when one has to decide. Afterwards it is easy to blame others for their wrong decisions,even though they had good intentions.
