While hysterectomy may help significantly reduce pain from #endometriosis, a new study published in Obstetrics & Gynecology found that patients under 30 who underwent laparoscopic hysterectomy for benign disease experienced much higher rates of regret, particularly due to loss of fertility (39% vs 13.4% in older patients). Younger patients were also more likely to face postoperative complications and need further surgery. YES. New study showing younger patients have higher rate of regret after hysterectomy. But authors wisely also explain: "However, it must be emphasized that these data should not preclude the option of hysterectomy when deemed appropriate, because a strong majority of patients in both age groups were satisfied with their decision. Instead, focus should be placed on shared decision making and respect for patient autonomy when counseling individuals about procedures that result in permanent sterilization." I dunno why we even study regret anymore. Happy to answer any questions. Here is a real recent case illustrating new approaches to patient management. An 11yo girl has elective abdominal surgery. She develops signs of MH after about one hour during surgery. She is treated successfully with dantrolene. Not the end of the story. Taken to ICU as per protocol and CK found to be 30,000! Very abnormal. Dantrolene contued as CK declines. Consults to genetics and neurology. Genetic test reveals likely pathogenic DNA variant. She then states that she does not play sports because she gets overheated. The grandmother reveals for the first time that she was evaluated for chronically elevated CK but asymptomatic. Follow up: patient has a myopathy and should be followed. All relatives should be tested for the DNA variant and counseled, the primary care providers need to be informed and educated, the case should be reported to the MH Registry(namhr.org). MH is much more than an isolated anesthetic episode. Like all inherited disorders needs to be handled by several different specialties coordinating care and management.
