In light of Lord Darzi's recent report on the NHS, it's crucial to reassess how community and primary care are structured. This report presents a significant opportunity to discuss whether community trusts should be reorganised, with substantial parts of their portfolio managed by General Practice. Many of these areas are under block contracts, that commissioners seem unable to break up, and that are very difficult to show value for money.  GPs have consistently demonstrated their deep understanding of their patients and communities, enabling them to swiftly adapt resources and services to meet patient needs. Unlike other healthcare sectors, General Practice has a track record of not operating at a deficit, highlighting its efficiency and effectiveness in resource management. This moment calls for a constructive dialogue on leveraging the strengths of General Practice to enhance community care, ensuring that services are both patient-centered and economically sustainable. Our CEO and COO, Jay Rebbeck and Meera Chadha Moynihan had a very enjoyable few days last week engaging with fellow advocates for Person Centred Value-Based Healthcare #PCVBHC. It was grounding to hear first-hand stories of outcomes-focused care, where the patient's needs and wishes were at the heart of the care they received. Many of the themes discussed at the event were related to areas we've been championing for some time with our commissioning work. We focus on co-design and outcomes rather than just outputs, so it was great to hear the breadth of examples of this working in practice. Two things we'd love to see built into the conversation: 🌱 Can we shift the language from needing such a long acronym to knowing that "care" as a stand-alone word epitomises the focus on patient needs, clinical outcomes, and good value as a standard? 🌱 How can we ensure that care is being designed and delivered to increase access so that under-served, reluctant, and remote populations have equitable outcomes, are heard, and their needs are put at the centre of delivery? This is particularly true for Aboriginal & Torres Strait Islander populations. Connecticut hospitals and health systems are building a healthier state, and their many contributions to the community transcend hospital walls. Hospitals are developing opportunities to drive regional transformative change in community health, care delivery, and health equity. 🏥 Through community benefit programs that encompass a wide range of services, hospitals are collaborating with local organizations to address housing, food insecurity, transportation, diaper insecurity, and employment. Hospitals also provide free and discounted care, subsidize essential health services, and contribute to research -- investments amounting to $3.4 billion in 2022. This #NationalHospitalWeek, learn about some of the many programs that Connecticut hospitals and their partners have united to establish in CHA's "Community Benefit Report," one of three annual publications. The Commonwealth Fund released important findings during National Health Center Week highlighting the challenges Community Health Centers face as they continue to provide high-quality care to a record 32.5 million people. The survey of 737 heath center leaders described persistent workforce shortages, lack of financial resources, and Medicaid redeterminations. Yet, health centers are doing more than ever – increasing the use of telehealth services, providing more substance use treatment, and screening for social needs. Highlights include: . Nearly all Community Health Centers offer timely appointments (88%) and expanded hours for patients to receive care (93%). . Two out of three (65%) of health centers report that lack of financial resources to address unmet needs was a major challenge. . Three out four health centers (77%) are experiencing a shortage of mental health providers. . Health centers report growing workforce challenges with over 70 percent experiencing shortages of primary care physicians (70%), nurses (70%) or enabling services providers (69%). This important narrative demonstrates the resilience of health centers after working on the frontlines of a devastating global pandemic and the critical importance of investing in primary care. Today was the NHS PEP kickoff. This program taps the expertise of patients, caregivers and advocates to evolve our healthcare system. It's based on the belief that lived experiences should steer meaningful change. I met incredible people building innovation projects addressing significant gaps in Healthcare. Their stories highlighted how grassroots initiatives by passionate locals are vital to filling health needs. The CEP recognises that top-down approaches can't address local nuances that impact health equitably. The people using services daily have crucial insights, and their voices and choices matter. When communities are empowered to lead change, it can transform lives across the UK's diversity. This programme is just what we need at this stage in building Eli Health.
