Please save the dates 4/10 & 4/11 2025 and join us at PACH’s inaugural conference, where we will CONVENE, CONNECT, and INNOVATE to address critical challenges in community health. While rooted in Ohio, this event aims to engage stakeholders from across the country, creating a space to share best practices and explore solutions applicable to both rural and urban communities nationwide. Key sessions will focus on social determinants of health (SDOH), including food insecurity, housing instability, transportation needs, utility difficulties, and interpersonal safety. A central focus will be workforce development, highlighting strategies to grow, support, and sustain Community Health Workers (CHWs) and strengthen Community Health Networks. Discussions will explore payment models, technology to support community and system collaboration, and barriers to workforce growth. Attendees will engage with national and local experts to explore growth opportunities for CHWs, non-traditional partnerships, and tipping points needed to drive impactful change. With targeted networking opportunities and presentations on innovative solutions, this conference is designed to inspire action, foster collaboration, and promote health equity across Ohio and the nation.
