Patient-centered care is a cornerstone in healthcare, but its implementation poses challenges. As an OT, I recognize the importance of assessing individuals in their own environment to truly understand their functional capabilities. Research shows that Health transitions in older adults depend on effective self-management, and health care providers need to provide care that promotes sustained and effective self-management in older adults rather than promoting patient dependence. A key challenge in transitional care is the need to provide patients with medical care that is adapted to independent, self-directed needs, i.e., their own perceived, rather than professional-defined needs (Older persons' experiences of adapting to daily life at home after hospital discharge: a qualitative metasummary, Hestevik CH, Molin M, Debesay J, Bergland A, Bye A.). The NHS has approved the rollout of Mounjaro, starting in March 2025. It’s a positive step, but with the rollout expected to take 12 years to reach all eligible patients, the scale of the challenge is clear. Our Medical Director, Dr. Bryony Henderson, spoke with BBC News last week on the issue: “Around 42,000 patients are expected to benefit from the initial 18-month rollout of Mounjaro, but that’s a tiny percentage of the millions eligible. The unprecedented step of giving the NHS 12 years to introduce it also signals that the NHS can’t tackle the UK’s obesity challenge alone. The health risks from obesity start at a much earlier point than a BMI of 35, and initially, this treatment will only be offered to those with a BMI of 40+. We recognise how important it is to make these innovative obesity treatments more accessible: more than 3.4 million people living in England are eligible for this help and support. Regulated, private healthcare providers provide a safe and clinically responsible route to treatment, reducing pressure on the NHS and improving public health.
