🌞 I'm taking advantage of the holidays to share with you the story of moments or people who have contributed to advancing hemodialysis and allowing millions of people to live despite chronic renal failure. 𝗧𝗵𝗲 𝗜𝗻𝗻𝗼𝘃𝗮𝘁𝗼𝗿𝘀 𝗪𝗵𝗼 𝗠𝗮𝗱𝗲 𝗞𝗶𝗱𝗻𝗲𝘆 𝗧𝗿𝗮𝗻𝘀𝗽𝗹𝗮𝗻𝘁𝘀 𝗣𝗼𝘀𝘀𝗶𝗯𝗹𝗲 In earlier episodes, I presented the men and innovations that made long-term hemodialysis possible. Now, let's explore how medicine came to make kidney transplants possible. In 1954, a year marked by the first successful ascent of Mount Everest by Sir Edmund Hillary and Tenzing Norgay, a groundbreaking innovation in medicine was born. Meet the doctors and scientists who refused to limit themselves to managing patients with kidney failure and sought to cure them, embarking on the quest for transplantation. The journey began with early attempts at kidney transplantation in France using reins from guillotined prisoners, which were unsuccessful due to immunological rejection. In 1953, the story of Marius Renard, a young French carpenter who received a kidney transplant from his mother but died three weeks later due to rejection, highlighted the challenges of organ rejection. The first successful kidney transplant was performed by John Merrill and 𝘑𝘰𝘴𝘦𝘱𝘩 𝘔𝘶𝘳𝘳𝘢𝘺 (1919 – 2012, portrait on the left in the illustration) on identical twins in 1954. This groundbreaking procedure demonstrated the feasibility of organ transplantation and paved the way for future advancements. 𝘑𝘰𝘴𝘦𝘱𝘩 𝘔𝘶𝘳𝘳𝘢𝘺 continued to make significant contributions to the field, including the first successful allograft and cadaveric renal transplant. A significant breakthrough came in 1958 when Jean Dausset discovered the HLA groups, which played a crucial role in understanding and preventing organ rejection. This discovery allowed for better matching of donors and recipients, improving the success of transplants. The development of immunosuppressive drugs like cyclosporine further enhanced the success of kidney transplants, enabling transplants from unrelated donors and significantly increasing survival rates. Today, the median survival with a kidney transplant is 16 years, highlighting the significant advancements made in the field of transplantation. Medical professionals at Massachusetts General Hospital reported on Thursday that Slayman is progressing well post-surgery and is anticipated to be discharged soon, offering promising prospects for the feasibility of such #transplants in the future. Historically, such endeavors were hindered by the immediate rejection of foreign #animaltissue by the human immune system. Currently, over 100,000 individuals are on the national waiting list for organ transplants, with a predominant need for kidneys. Virtual Crossmatching in Kidney Transplants: A Critical, Yet Overlooked, Tool in Modern Transplantation Kidney transplantation remains one of the most effective treatments for #ESRD, offering patients a chance for a better quality of life compared to long-term #dialysis. Yet, one crucial aspect of the #transplant process—virtual crossmatching—often goes unrecognized, despite its significance in improving transplant outcomes. This is an area that major organizations like the, Fresenius Medical Care, and DaVita Kidney Care have yet to fully emphasize in their #advocacy and education efforts. Virtual #crossmatching is a laboratory technique that predicts the compatibility between a donor and recipient before an actual transplant. Unlike traditional crossmatching, which requires the physical mixing of donor and recipient blood, virtual crossmatching relies on advanced immunological testing and data analysis to assess the likelihood of a successful transplant. It involves comparing the recipient’s antibody profile with the donor’s human leukocyte antigen (HLA) typing to predict potential rejection risks. ➡️The Significance of Virtual Crossmatching 🧬Speed and Efficiency ⚕️Improved Transplant Outcomes 🩺Broader Donor Pool Recent studies highlight the growing importance of virtual crossmatching in kidney transplantation. A 2021 study published in the American Journal of Transplantation found that virtual crossmatching significantly reduced the time to transplantation and improved one-year graft survival rates compared to traditional crossmatching methods. The study also emphasized the potential of virtual crossmatching to reduce sensitization in recipients, thereby lowering the likelihood of future rejection (Montgomery et al., 2021). Despite the clear benefits, virtual crossmatching remains underutilized and underrepresented in the broader discussions on kidney transplantation. Major organizations like the National Kidney Foundation, Fresenius Medical Care, and DaVita Kidney Care focus heavily on dialysis and the general aspects of transplant surgery, but they often overlook the importance of pre-transplant procedures like virtual crossmatching. Increasing awareness about virtual crossmatching is not just a matter of education—it’s a matter of improving patient outcomes and expanding access to kidney transplants. Patients need to be informed about all aspects of the transplant process, including the tools that can enhance their chances of a successful transplant. By integrating virtual crossmatching education into their advocacy efforts, organizations can ensure that patients are better prepared and more likely to receive a successful transplant. "3rd Consultative Meeting of the KP Institute of Haematology, Transfusion, and Transplantation" The third consultative meeting of the KP Institute of Haematology, Transfusion, and Transplantation (Proposed) was held on April 20, 2024, at the Pakistan Thalassemia Welfare Society (PATHWEL) in Rawalpindi, following the directive of Dr. Amjad Ali Khan, MNA, and Advisor to the Chief Minister, KP. The main goal of this third meeting, which followed two earlier ones on March 25, 2024, held in RBC Peshawar and the KP Chief Minister's Office, was to develop and carry out a cost-effective plan for establishing the first bone marrow transplant unit in the public sector in Khyber Pakhtunkhwa (KP). The meeting, presided over by Maj. Gen. (R) Prof. Parvez Ahmed, former Commandant of the Armed Forces Bone Marrow Transplant Centre, was attended by Brig. (R) Dr. Kamran Mehmood (Consultant Physician and BMT Specialist), Dr. Khalil ur Rehman (Consultant Clinical Haematologist), Dr. Usman Waheed (Technical Expert, IHRA), and Dr. Noor e Saba (Consultant Haematologist, RBC Peshawar). Prof. Parvez Ahmed proposed the establishment of a centralized unit to address various medical needs encompassing blood transfusions, bone marrow transplants, haematology, oncology, and diagnostics, leveraging existing infrastructure from nearby tertiary care hospitals (such as laboratory, radiology, ICU, cardiology, and pharmacy facilities). It was deliberated that a Board of Governors needs to be notified to oversee this project. Other members may be selected after consultation. Technical experts, public health specialists, both national and international, may be nominated. Three main areas were identified as crucial for a sustainable programme: infrastructure, efficient human resources, and financial backing. Prof. Parvez Ahmed appreciated the commitment of the Peshawar RBC team for taking the lead in this project and offering all possible support, including the premises of RBC as a pilot for the proposed project. A committee of experts will be responsible for filtering deserving patients and allocating funds accordingly, following the model of Shaukat Khanum Cancer Memorial Hospital. The proposed project aims to strengthen its capacity to train medical experts and paramedic personnel, with initial training sessions to be facilitated at Pathwell Hospital in Rawalpindi under the supervision of Prof. Parvez Ahmed. The meeting concluded with a consensus on the immediate initiation of action items, including cost analysis, HR requirements assessment, and the design of a comprehensive project proposal (PC-1) for the establishment of a bone marrow transplant unit in KP, emphasizing sustainability, capacity building, a patient-centric approach, and the integration of thalassaemia prevention initiatives. All stakeholders agreed to collaborate further on the implementation process, with action items assigned for immediate follow-up.

Improving Access

By now, most people have heard that Elle Mcpherson has eschewed medical advice in favour of a “holistic approach” to her breast cancer diagnosis.  When someone with a public profile makes such a controversial statement, it will generate headlines & it would be easy to label her interview as utterly irresponsible (which was my first reaction to be honest). However, the reality is that she has a considerable influence & her words have an impact. There may be cancer patients who are now considering their future treatment options as a result so I think it is vital to reiterate a few simple facts from an oncologist’s perspective : ·  Every cancer diagnosis is different & the treatment for patients with the same cancer can vary considerably.  It is not a “one size fits all” approach. ·  Every patient has a right to make decisions about their health, based on their own values & beliefs. Patient autonomy is key to good medical care & it is respected by medical professionals - even it is not in line with medical guidelines. ·  However, decisions about your health need to be made when you are in full knowledge of the facts. A key part of the conversation with your oncologist at diagnosis is about treatment options & what benefit/risk these treatments will have in your particular case. This is an open & honest dialogue – your oncologist is never going to force you to have treatment – but they will explain how conventional treatment can reduce the risk of cancer recurring in your particular case. These treatments have been rigorously tested through clinical trials (rather than advice from a supermodel). ·  The majority of patients choose to be guided by clinical evidence & advice from their oncologists. However, if you do want to consider alternative therapies, it is really important to talk to your oncologist about this. They can help you understand what these therapies entail, the potential risks/benefits involved and how it compares to conventional treatment. ·  Just because you choose “conventional” therapy doesn’t mean you can’t have complementary medicine - It is not an “either/or” approach.  We, as oncologists, often recommend complementary therapies to help patients through their treatment. Treatments like acupuncture (which can help with distressing hot flushes) & yoga to mention but two that can be used in conjunction with conventional therapies to improve our patient’s experience. However it is key that you talk to your oncologist to make sure it is the right decision in your situation. Our role of oncologists is to support our patients throughout their cancer journey & help them make the best decision when they are at their most vulnerable. We have a duty to recommend treatments which show clear clinical benefit. This approach will never generate sensationalist headlines but it is done with our patients' best interests at heart. 📢 Happy to share our new original research 🇨🇵 just published in the New England Journal of Medicine NEJM Group that offers new insights into #kidney #allograft #rejection and presented today in San Diego American Society of Nephrology #KidneyWeek. -> Over 16,000 biopsies from 30+ centers 🌎 were included, investigating how the newly introduced phenotypes in the 2022 Banff Classification, including probably antibody-mediated rejection and microvascular inflammation, DSA-negative, and C4d-negative, impact disease progression and long-term allograft outcomes. 🔍 Of the 16,293 biopsies studied, 788 were identified with the newly-recognized microvascular inflammation phenotypes, with 641 of these previously classified as nonrejection. Our findings revealed that these phenotypes are associated with varying risks of allograft outcomes. 📊 Notably, patients with microvascular inflammation without an antibody-mediated response (DSA-negative, and C4d-negativee) had an increased risk of graft loss compared to patient without rejection. 📃Scientific paper: Trends in kidney transplantation and living donor nephrectomy in Germany: a total population analysis from 2006 to 2021 Abstract: Purpose To analyze recent trends of surgical access routes, length of hospital stay (LOS), and mortality in kidney transplantation (KT) and living donor nephrectomy (LDN) in Germany. Materials and methods We studied the nationwide German hospital billing database and the German hospital quality reports from 2006 to 2021. Results There were a total of 35.898 KTs. In total, 9044 (25%) were living donor transplantations, while 26.854 (75%) were transplantations after donation after brain death (DBD). The share of open LDN decreased from 82% in 2006 to 22% in 2020 (− 4%/year; p  < 0.001). The share of laparoscopic LDN increased from 18% in 2006 to 70% in 2020 (+ 3%/year; p  < 0.001). The share of robotic LDN increased from 0% in 2006 to 8% in 2020 (+ 0.6%/year; p  < 0.001). Robotic-assisted KT increased from 5 cases in 2016 to 13 procedures in 2019 ( p  = 0.2). LOS was shorter after living donor KT, i.e., 18 ± 12.1 days versus 21 ± 19.6 days for DBD renal transplantation ( p  < 0.001). Moreover, LOS differed for open versus laparoscopic versus robotic LDN (9 ± 3.1 vs. 8 ± 2.9 vs. 6 ± 2.6; p  = 0.031). The overall in-hospital mortality was 0.16% ( n  = 5) after LDN, 0.47% ( n  = 42) after living donor KT and 1.8% ( n  = 475) after DBD KT. Conclusions There is an increasing trend toward minimal-invasive LDN in recent years. Overall, in-hospital mortality was low after KT.
